About the Role

We are seeking a skilled Licensed Practical Nurse to join our team as a Community Care Specialist. In this role, you will provide high-quality nursing care to patients in their homes, working with patients of all ages and complexity of care needs.

Key Responsibilities

  • Participate in scheduled visits to assess patient needs and provide care within your scope of practice
  • Provide safe and ethical nursing care, using appropriate assessment tools and techniques
  • Develop and implement care plans, monitoring patient progress and adjusting as needed
  • Engage patients in their care, building rapport and trust through a collaborative approach
  • Maintain accurate and up-to-date patient records

Requirements

  • Registration with the NSCN as a Licensed Practical Nurse in good standing
  • Valid driver's license and access to a vehicle
  • Experience in community care is an asset
  • Reliability, flexibility, and a willingness to travel to patient residences
  • Experience working with vulnerable populations and a passion for making a difference in the community
