Job Summary

We are seeking a skilled Licensed Practical Nurse to join our hemodialysis team at Fraser Health. As a key member of our interdisciplinary team, you will provide high-quality patient care and contribute to the development of individualized care plans.

Key Responsibilities
  • Assess, plan, implement, and evaluate patient care during the pre-dialysis, intra-dialysis, and post-dialysis periods.
  • Establish and maintain therapeutic relationships with patients and their families through effective communication and interpersonal skills.
  • Contribute to multidisciplinary care planning meetings and family conferences to ensure patient-centered care.
  • Perform patient care duties in collaboration with the Registered Nurse and other members of the nephrology team.
  • Advocate for patient interests and needs, escalating care as required for changes in patient condition.
Requirements
  • Grade 12, plus graduation from an approved program of Practical Nursing, and successful completion of post-basic education through an in-house program offered by a health authority affiliated with BC Renal.
  • One year recent related experience working as a Licensed Practice Nurse in acute care, or an equivalent combination of education, training, and experience.
  • Current full practicing registration with the British Columbia College of Nurses and Midwives (BCCNM) and current certificates in CPR Level C or equivalent and First Aid.
