Supervisor – Care Coordinating (Visiting Nursing Program)

Location: Head Office
Position Type: Full-Time, Regular
Hours: Days (8:00 AM – 4:00 PM or 9:00 AM – 5:00 PM), 75 hours bi-weekly
On-Call: Rotating responsibilities
Salary: $23.53 – $36.56 per hour (Non-Union Wage Grid)

St. Joseph’s Home Care is seeking a Supervisor – Care Coordinating to join our Visiting Nursing Program. What You’ll Do

- Supervise and support Visiting Program Assistants and nursing staff.

- Coordinate scheduling, staffing, and daily operations for efficiency and quality.

- Provide clinical guidance, conduct audits, and assist with care planning.

- Act as a key liaison between nurses, management, and clients.

- Promote teamwork, integrity, and compliance with best practices.

What We’re Looking For

- Education: RN or RPN diploma; member in good standing with the College of Nurses of Ontario.

- Experience:

- 3–5 years in nursing or care coordination (required)

- 1–3 years in a supervisory role (preferred)

- Skills: Strong leadership, problem-solving, and communication abilities; proficiency in MS Office and electronic medical record systems.

- Background: Community, home care, acute care, or LTC experience is an asset.
