Responsibilities

- Provide physiotherapy assessment and treatment in community settings, including cardiovascular and pulmonary rehabilitation as applicable.
- Develop and implement individualized treatment plans and education for clients and families.
- Collaborate with interdisciplinary teams and community partners to support client goals.
- Document progress and outcomes, and adjust plans as needed.
- Maintain professional practice within scope and adhere to relevant standards and regulations.

Qualifications

- Registered Physiotherapist with applicable provincial college (e.g., CPO, Ontario).
- Experience in community/home care settings preferred.
- Strong communication, problem-solving, and compassionate patient-centred approach.
