What You’ll Be Doing
As an OT on this integrated care team, you’ll:

- Conduct in-home assessments to evaluate cognitive, functional, and psychosocial abilities.

- Collaborate with an interdisciplinary team of physicians, nurses, PTs, SLPs, and social workers to develop personalized care plans tailored to patients’ unique goals.

- Recommend, source, and adapt assistive technologies to support patient safety and independence.

- Provide education to caregivers on personal care, safe transfer techniques, and home safety best practices.

- Maintain accurate clinical records, including assessments, care plans, progress reports, and discharge documentation.

- Participate in team huddles and case conferences to coordinate and align patient care strategies.

Qualifications You Bring

- Master’s Degree in Occupational Therapy

- Current registration in good standing with the College of Occupational Therapists of Ontario (COTO)

- 3–5 years of clinical experience in occupational therapy (preferred)

- Proficiency in assessment, goal setting, and evidence-based treatment planning

- Knowledge of assistive devices, adaptive equipment, and environmental modifications

- Excellent communication skills and ability to work as part of an integrated care team

- Reliable vehicle and valid Ontario driver’s license
