SE Health is proud to partner with CHEO to provide care and support to children and families who need home and community care services. The Pediatric Nurse is an important member of the child’s care team and will make a meaningful difference in the life of a child.

SE Health also provides palliative care services that support both the individual facing a life-threatening illness and his/her family. Improving the quality of life is a key focus of the care that is delivered by our team of nurses working with other professionals within the health care system (personal support workers, physicians, chaplains, therapists, hospice volunteers, etc). Recognizing that individuals living with illness are also faced with many needs, our staff is able to integrate the physiological, psychological, socio-cultural, and spiritual aspects of care to assist those coping with their situation. Our community-based palliative care team offers a support system to help people live as actively as possible until death and to assist families during the person’s illness and during bereavement.

Responsibilities:

- Complete comprehensive client assessments, develop care plans, implement and evaluate care based on best evidence/practices and on client’s unique needs.
- When designated as a primary provider for a client caseload, you will be the point of care lead accountable for their care while working in collaboration with other care team members and the support functions.
- Support the client through care transitions by collaborating and connecting the client with community support services in their local community.

Requirements

- Current registration with the College of Nurses of Ontario
- Canadian Nurses Association (CNA) specialty certification an asset
- Enthusiasm and love of community care and passion for evidence-informed practice & excellence in care
- Hospice Palliative care experience and ongoing continuing education is preferred
- A current CPR certificate is required
- A valid driver’s license and vehicle (travel required)
- Updated immunization records, clear criminal background check, and Vulnerable sector check are required upon hire.
- A willingness to attain required SE Health certifications.
- This is a physically demanding position and requires hand, arm, and shoulder dexterity, sensory ability, lifting to a maximum of approximately fifty-five (55) pounds, walking, climbing stairs, standing, bending, reaching, and frequent movement.
