Apply today to join our team As a Rehabilitation Assistant with VCH you will make your impact by: Function under the operational supervision of the Manager or Team Leader and the work direction of a licensed designated occupational therapist or physiotherapist to work as a member of the health care team.Provide assigned physical / occupational therapy therapeutic interventions.Assist in modification of OT/ PT care plans and facilitate transition to community programs in order to enable clients to maximize potential for independence.Provide functional direction to family, report relevant observations of client's condition in writing or by utilizing basic computer literacy.Manage equipment safety and inventory.ABOUT HOME HEALTH PROGRAMS: Home Health programs provide a range of health care and support services to people living in the community who have acute, chronic, palliative or rehabilitative health care needs. THE CLIENT POPULATION:Home Health clients typically have on-going health issues and/or complex personal and/or social issues, requiring care planning and coordination of appropriate resources. Clients include frail elderly, younger clients with disabilities, and adults with chronic illnesses requiring supports to maintain/promote independence. Robert and Lily Lee Family serves the Grandview, Woodlands and Commercial Drive neighbourhoods, and Pender serves Gastown, Chinatown, Strathcona and the Downtown Eastside neighbourhoods. Qualifications Education & ExperienceCompletion of a recognized Diploma level Rehabilitation Assistant Program with Occupational and Physical therapy components plus one (1) year's recent, related experience working with target population.Valid BC Drivers License. Local area travel requires the use of a personal vehicle for which mileage will be reimbursed.Knowledge, Skills & AbilitiesAbility to communicate effectively both verbally and in writing.Computer literacy skillsAbility to work collaboratively with others.Physical ability to carry out the duties of the position.Ability to organize and prioritize work.Ability to observe and recognize changes in clientsAbility to analyze and solve problems.Ability to establish and maintain rapport with clientsAbility to motivate clients and to encourage progression through therapeutic goalsDemonstrated ability to provide self-care instruction and education Closing Statement The hours of work including days off and work area may be subject to change consistent with operational requirements and the provision of the Collective Agreement and applicable statutes.
