Position Summary

We are seeking a compassionate and highly organized Care Coordinator to support clients in navigating their care journeys. In this role, you’ll manage internal and external coordination - handling referrals, follow-ups and care transitions – while also building trusted relationships with each client.

This role is ideal for someone with a clinical or health navigation background who brings a detail-oriented mindset and a passion for advocacy and continuity of care.

Key Responsibilities

- Act as a liaison between clients, specialists, hospitals, labs, and community providers
- Schedule, track, and follow up on specialist referrals, diagnostics, and procedures
- Support individualized care plans and participate in interdisciplinary case reviews
- Conduct follow-up calls and occasional external visits as needed
- Provide client education on diagnoses, medications, and care plans
- Ensure EMRs are updated with accurate appointment, referral, and outcome details
- Support insurance-related processes such as pre-authorizations and verification
- Serve as the main point of contact for care-related inquiries and coordination

Qualifications & Experience

- Registered Nurse (RN) or Registered Practical Nurse (RPN) in good standing with the College of Nurses of Ontario
- Bachelor of Science in Nursing (BSN), or equivalent health navigation background
- Other relevant backgrounds may include: patient navigator, medical social worker, health coach, or private care concierge
- Minimum 3 years of experience in care coordination, case management, community health or outpatient support
- Strong understanding of Ontario’s healthcare system, pathways, and provider networks
- Exceptional communication, documentation, and organizational skills
- Proficiency in EMR systems and digital clinical workflows
- Additional training in chronic disease management is an asset
