JOB SUMMARY: The Licensed Practical Nurse ( LPN) provides planned individualized care to residents to promote quality of life, within the Scope of Practice of LPN’s, under the direction of the Registered Nurse ( RN) or Registered Psychiatric Nurse ( RPN).

Reports to the Care Coordinator or Designated Supervisor

DUTIES AND RESPONSIBILITIES

- Collaborates with the Registered Nurse.

- Assist residents with or fully provide care as needed to meet activities of daily living, i.e. nutritional needs, personal hygiene, bathing, skin care, hair and nails, bathing, toileting and dressing. Oversees prescribed exercise routines.

- Lifts, transfers, ambulates and transports residents utilizing mechanical aides such as lifts, wheelchairs and walkers.

- Carries out nursing procedures according to facility standards such as:

- Administration of oral, tropical, and injectable medications as per facility policy.

- Applying and changing sterile and clean dressing.

- Taking and recording vital signs

- Glucometer readings

- Ostomy care

- Collecting and testing urine

- Administering and monitoring oxygen

- Administering suppositories and enemas

- Enteral feedings

- Other treatment appropriate to the LPN Scope of Practice

- Works collaboratively with the RN by:

- Responding to emergency situations.

- Participates as a member of the interdisciplinary team by taking part in the development, implementation, assessment and evaluation of resident care plans and profiles including regular review and updating.

- Documenting and attending resident care conferences.

- Reporting significant changes in a resident’s conditions.

- Assesses physical, psychosocial and cognitive status of residents and report changes to the RN such as:

- Conducting wound assessments

- Assessments of the effectiveness of the treatments.

- Assessment of pain.

- Head to toe assessments.

- Falls assessments

- Other assessments appropriate to the LPN Scope of Practice

- Participates in and assist with the resident’s social activities, when appropriate.

- Takes part in the admission of new residents and orientates resident and family members to facility.

- Documents relevant observations, treatments, medications and activities as per facility policy.

- Transcribes Doctor’s orders, books appointments and carries out other clerical duties as needed.

- Uses, cleans and stores nursing equipment and reports unsafe equipment or environment to the appropriate department.

- Responds to unsafe environmental factors in resident’s room and in the lodge by correcting the situation, alerting the staff and residents as appropriate, and /or alerting the supervisor, maintenance department or the other manager.

- Attends staff development programs to maintain current knowledge of nursing practices.

- Performs other related duties as assigned.

Education and Experience

- Licensed to practice nursing in the Province of British Columbia under the College of Licensed Practical Nurses of BC.

- Current CPR certificate.

Knowledge

Incumbent requires a practical understanding of licensed practical nursing theory, assessment skills and procedures, together with the theoretical and clinical knowledge of administering medications.

Skills and Abilities

- Able to assess and monitor client‘s physical and psycho-social status and report changes to the registered nursing staff.

- Able to assess, plan, implement and evaluate nursing care.

- Able to prepare and maintain accurate records and reports.

- Able to communicate effectively in English, both written and oral, give direction and organize work schedule.

- Able to work collaboratively with other care team members.

- Able to establish and maintain good interpersonal relations with the residents, staff, visitors and volunteers.

- Be responsible and accountable for the nursing care and services they provide as determined by the LPN Standards of Practice.

- Able to meet the physical requirements of the job which includes lifting, bending, walking and operating related equipment.

- As a result of qualities and attitudes of the LPN, is able to provide resident care that respects and maintains the spirit, dignity and individuality of the person.
