Job Summary

We have the perfect Relief Full‑Time opportunity for a Clinical Pharmacist to join our team at BC Children’s & Women’s Hospital located in Vancouver, BC. This relief role is available until September 2026 or until return of incumbent.

Responsibilities
1. Consult with physicians, nurses and other health care providers to develop and discuss therapeutic plans appropriate for each patient based on chart review and assessment of clinical responses. Ensure appropriate documentation is maintained in the patient health record.
2. Individualize patient medication therapy by referencing standard literature, reviewing patient charts and laboratory test results, meeting with the patient and health care team, and modifying or developing therapeutic plans to ensure medication therapy is consistent with the patient’s needs.
3. Provide patient medication counselling regarding drug therapy, review reference literature, summarize information, meet with patients and families to provide and explain information and plans, demonstrate procedures, techniques and use of equipment, and assess understanding.
4. Assist the health care team in developing drug therapy guidelines for individual disease conditions by reviewing current literature, compiling and analyzing information, providing suggestions and recommendations, and monitoring effectiveness.
5. Participate in ward rounds, program team meetings and provision of pharmacy services at the unit level to enhance patient care.
6. Act as department resource person in the assigned area of responsibility. Support staff by providing detailed drug information on issues related to drug therapy in diseases treated within the program.
7. Contribute items such as case studies and review articles to professional journals by identifying topics, gathering and analyzing data, reviewing literature, writing and submitting articles for publication.
8. Provide instruction and/or supervision to pharmacy students and residents by preparing educational material, demonstrating procedures, lecturing, supervising during patient care activities and providing input into performance evaluations.
9. Participate in quality improvement activities, including reviewing current practices, performing quality assurance audits, identifying problems and recommending changes.
10. Perform drug distribution duties, including interpreting medication orders, dispensing medications, ensuring accuracy and documenting activities in the patient health care record.
11. Review prescriptions for therapeutic suitability by referring to standardized references and laboratory data.
12. Communicate with physicians, patients and other health care team members for prescription clarification and problem resolution, and document outcomes.
13. Ensure medications are efficiently and accurately dispensed and/or compounded and labeled, and that all necessary dispensing records are completed.
14. Participate in data collection for medication use evaluation projects and maintain inventories, requisition drugs, monitor expiry dates and report procedural errors.
15. Participate in clinical drug trials by dispensing medications, completing documentation and reviewing adverse reaction reports.
16. Participate in implementation of medication safety initiatives and quality control activities.
17. Participate in staff meetings, committees and other programs as required; identify opportunities to improve processes and refer to supervisor.
18. Maintain records for narcotic and controlled drugs, special access program drugs, and departmental statistics in accordance with legal and departmental standards.
19. Prepare reports on various activities, gather and record information into approved formats, account for variances, and update files and spreadsheets.
20. Perform other related duties as assigned.
Qualifications
Education and Experience
Current registration as a Pharmacist with the College of Pharmacists of British Columbia. Completion of an accredited Hospital Pharmacy Residency Program or an equivalent combination of education, training and experience.
Skills and Abilities
- Broad knowledge of organization policies, procedures and standards of care.
- Knowledge of evidence‑based clinical pharmacy practice.
- Knowledge of medication use evaluation.
- Knowledge of research processes and methodology.
- Knowledge of drug distribution processes in a hospital setting.
- Ability to apply clinical pharmacy knowledge and make decisions with a minimum of supervision.
- Ability to communicate effectively both verbally and in writing.
- Ability to establish and maintain effective working relationships with other health practitioners, patients, family members and the public.
- Ability to organize and prioritize work.
- Ability to operate related equipment.
- Physical ability to carry out the duties of the position.
