**Terms**:2 year contract with potential for permanent

**Salary/rate**: $60,000/year - 35 hours a week

**About us**:
Thrive Counselling is a family service agency serving the Halton Region. We provide a variety of counselling and other services to support our community and increase their well-being and safety.

We are currently launching a new service hub to assist individuals and families impacted by abuse and violence, including Intimate Partner Violence, Sexual Violence and Human Trafficking.

**About the role**:
This role will act as the primary point of contact for clients such as registering for services, discussing service options, facilitating referrals and service connection, requesting information and providing ongoing case management support. The Navigator will oversee the client’s journey through phases of service including the coordination of appropriate support and facilitating relationships between the client and appropriate service providers/partners.

**Responsibilities**:

- Coordinates and navigates systems of care that will support the client to achieve their care goals.
- Assesses emerging circumstances identified by the client that risk destabilizing their safety and well-being.
- Responds to client inquiries regarding services and supports.
- Conducts initial intake to determine client needs.
- Conducts therapeutic, trauma-informed risk assessment and safety planning
- Facilitates referrals to on-site and off-site partners, agency programs, other professionals, agencies, services, or community resources appropriate to clients' identified needs
- Works closely with hub partners to ensure coordinated, responsive and timely service for clients
- Develops and maintains collaboration and partnerships, liaising, consulting and advocating with community service providers on behalf of the clients as appropriate
- Processes and maintains internal and external referrals
- Develops and implements program evaluations and monitors ongoing program performance.
- Provides ongoing case management, support and follow-up for clients.
- Supports outreach efforts as needed to promote an awareness and understanding of the programs and services provided by the hub
- Maintains careful, accurate records and client case files of all relevant information.
- MA level Degree in Social Work Health or Social Sciences field; or equivalent education/experience
- Experience or knowledge of gender-based violence and community-based resources
- Excellent interpersonal, oral & written communication skills
- Proven ability to work collaboratively
- Fluency in another language is an asset

**Salary**: $60,000.00 per year
