**Position Description**:
**Patient Care Information Coordinator, Complex Care/Transition to Adult Care**

**1.0 FTE (0.5 FTE Complex Care, 0.5 FTE TAC)**

We are currently seeking a Patient Care Information Coordinator to join our Complex Care team and our new Transition to Adult Care team. The Transition to Adult Care Program is a new initiative that seeks to support youth with complex, chronic illness and rare diseases, along with their caregivers, to transition from pediatric to adult care.

In this combined role, you will have a shared FTE between both programs and work collaboratively within both interdisciplinary teams to provide care to children who have medical complexity, as well as chronic illness and social needs. The focus of your role will be to partner with the patient and family, as well as teams, to coordinate care. You will be responsible for the patient scheduling process, and for supporting both teams with clerical and administrative functions in a proactive and patient-focused manner.

**Here’s What You’ll Get To Do**
- Facilitate communication and respond to requests for information from families and healthcare staff
- Collate clinical data and relay this information in a precise and timely manner to colleagues within and outside of the organization
- Coordinate appointment bookings and work with a variety of specialized care teams to facilitate timely, family-centered access to care
- Maintain appointment schedules on the computer system, and notify patients/families and clinical providers of updates and changes to appointments
- Arrange and coordinate teaM/Family meetings with multidisciplinary providers
- Answer phone calls and respond to requests for information from families and healthcare staff
- Support clinical staff in preparing, and organizing patient information and documentation
- Greet and orient patients and their families to the functions of the team
- Support the clerical staff with the registration and flow of patients within the clinic
- Initiate and assist in the coordination of family/patient and health care professional education sessions and events
- Communicate with inter-professional team members within and outside of the organization
- Act as the liaison for external hospital contacts
- Participate in interdisciplinary program rounds
- Pro-actively provide administrative support functions for the team, as required, including meeting minutes and mailings

**Here’s What You’ll Need**

**Essential Requirements**
- Diploma from a recognized Medical Administration program, or equivalent
- Minimum of 3 years clerical or administrative experience
- Scheduling experience
- Accurate keyboarding skills and strong working knowledge of Word, Excel, PowerPoint and Outlook
- Excellent problem solving skills that demonstrate an ability to anticipate potential issues and make suggestions
- Strong interpersonal, communication and organizational skills with both fellow colleagues and patients
- Ability to remain focused and prioritize tasks in a busy and high stress environment
- Dependability and good attendance

**Assets**
- Experience in a hospital environment
- Knowledge of medical terminology
- Experience with Epic (electronic health record)

**Employment Type: 1.00 FTE, Permanent, Full time.**
