**Job Overview**:Reporting to the Director, Mental Health, Addictions & Professional Practice the Quality Improvement (QI) Practice Lead will lead practice facilitation and quality improvement initiatives. This role will collaborate with team leads and their teams to leverage best practices, optimize processes, identify variances, ensure compliance with program standards, and support continuous quality improvement opportunities. The QI Practice Lead will be a systems thinker who is familiar with the tenants of the Patient’s Medical Home (PMH) and will strive to achieve them within a Community Health Centre Model of care delivery. This is a permanent, full-time position.

**Key Responsibilities**:

- Contribute to quality improvement strategies to improve patient outcomes and uphold program standards
- Engage with cross-functional teams to evaluate and optimize clinic processes and flow.
- Identify bottlenecks and inefficiencies and formulate improvement initiatives using reliable quality improvement methodologies.
- Engage proactively with project teams and clinical programs, contributing to process development, implementation, and refinement.
- Partner with clinics to successfully integrate team-based care.
- Collaborate with the Program Managers, and the Evaluation & Impact team to support program evaluation and to foster data-led leadership. Support team and program leads with analyzing and interpreting program-specific data to identify improvement areas and track outcomes
- Assess and Utilize data analysis (quality metrics, PREMs, PROMs) to:

- Assess and enhance patient care.
- Monitor and evaluate improvement efforts, recommending adjustments as needed.
- Create comprehensive reports and visual presentations to convey finding and program improvements.
- Present QI outcomes at internal and external meetings.
- Implement training to meet quality standards once identified.
- Actively contribute to program planning, evaluation, and performance tracking.
- Support compliance with program standards, guidelines, and regulatory mandates and accreditation standards.
- Stay updated on industry-leading practices and evidence-based guidelines.
- Possess expertise in electronic medical record management.
- Perform other duties as assigned.

**Qualifications and Experience**:

- Undergraduate degree in relevant health, medical or social science discipline
- Master’s degree preferred
- Demonstrated competence in leading quality improvement projects and programs
- Green Belt Certification, LEAN or equivalent training and experience
- PROSCI Change Management Certification preferred
- Demonstrated leadership skills and the ability to work effectively with people
- Ability to build a positive team and work culture
- Experience with Electronic Medical Records an asset
- Comfortable working with large sets of data, performing analysis and sharing results
- A satisfactory vulnerable sector criminal record clearance is required
- Demonstrated knowledge, understanding and commitment to the Truth and Reconciliation Commission of Canada: Calls to Action

**Work Environment and Hours**:

- 1.0 FTE (37.5 hrs/wk)
- Standard hours: 8:30 am - 4:30 pm, Monday-Friday; some flexibility in schedule is available
- Primarily works out of The Alex Community Health Centre: 2840-2 Ave SE, Calgary.
- Extended periods of sitting, with some standing and/or walking (sometimes up/down stairwells)
