Community Support Worker Purpose of Position:
The hourly wage posted is including $3;
- Permanent Wage Enhancement.

The community support worker is responsible for providing assistance and support to adults with a combination of development, physical and/or emotional challenges, by providing creative opportunities for people to learn and/or enhance skills relative to all aspects of daily living. In addition, helping people to develop and maintain personal relationships and helping people to become aware of and participate in community opportunities to the fullest extent possible.

Accountability:
Accountable to the individual supported and his/her family

Ensures that all activities are in accordance with the principles and philosophy of CLWD, its values and mission statement

Reports to and is accountable to the Program Manager in matters relating to performance, evaluation and service delivery. In absence of the Program Manager, is accountable to the Executive Director. Responsibilities:
to provide support to people receiving services from CLWD

advocate for inclusion in all areas of daily living, i.e. recreation, social, spiritual, educational, economic, employment and health services. Included with this is the ability to provide options, information and education to enable people to make choices, pursue their interests and to exercise their rights

assist and support people receiving services to maintain relationships with family and friends and to support the development of new relationships

to implement and adhere to instructions from specialists, behavioral consultants, speech pathologists, public health officials etc. on behalf of people receiving support

to provide periodic assessment of skills and abilities and implement teaching and/or support strategies as required

to assist people by physically lifting and transferring them when necessary while following guidelines for back care and using manual or mechanical lifting techniquesto assist with physical care and personal needs as required

to arrange for and provide transportation either with Agency or personal vehicles as required

to follow guidelines for financial accountability with respect to the supported individual’s funds and maintain current accurate records of the person’s accounts

be familiar with and follow medication procedures in accordance with Agency policies

be familiar with and carry out all policies and procedures of the Agency and adhere to the Mission statement

complete all mandatory trainings in a timely fashion

keep updated in the changing trends of the field of developmental services

use all personal protective equipment that is provided as required for the task to promote a healthy and safe work environment

Respond calmly and effectively to emergency situations. Capable of making decisions and problem solving in stressful situations

maintain a clean/safe work environment

assist with and in the orientation of CLWD staff and volunteers as required

Complete household duties such as cleaning, laundry, filing, etc.

Adhering to guidelines set forth by MCSS for purposes of Compliance Review(s)

Communication:
1. Communicate to all people with a sensitivity that respects the personal dignity and enhances the quality of life of people receiving support.

2. Adhere to and promote CLWD’s confidentiality and privacy policies.

3. Communicate any issues, problems or concerns regarding people receiving support, staff, volunteers, buildings, equipment or vehicles to the appropriate supervisory personnel in a timely fashion.

4. Participate in all relevant staff or other meetings on behalf of people receiving support, as required.

5. When communicating, ensure both paraverbals and body language reflect dignity and respect

Documentation/Record Keeping:
1. Be familiar with and carry out all policies and procedures of CLWD

2. Keep accurate, clear, concise records and complete reports on a timely basis.

3. Initiate accident/incident and serious occurrence procedures and provide follow up as required.

4. Adhere to procedures for time registration (ADP), vacation requests, action plans, purchase and travel expense requisitions, etc.

5. Maintain accurate records of individual financial accounts, medical appointments, and personal support plans, daily logs, MARS, etc.
