**ABOUT THE ROLE**
This position is responsible to plan, prepare and facilitate restorative care programs and focuses on the residents’ continuity of care to encourage independence and opportunities to meet the social, emotional, physical, and restorative care needs of residents. As a member of a multidisciplinary team, collaborates with contracted physiotherapist, the Restorative Care Aide and other professionals, this position is responsible for leading the implementation, evaluation, and revision of the therapeutic plan of care for residents for the restorative care program.
**WHAT YOU'LL BE DOING**
- Ensures that participating residents in the restorative care program receive care and assistance towards independence based on a restorative care philosophy to maximize autonomy a per the Resident Bill of Rights.
- Coordinates and integrates restorative care approaches into each participating resident plan of care to help maintain or improve their functional and cognitive capacities in all aspects of daily living to the extent of their abilities.
- Documents restorative plan of care to reflect residents needs, preferences and current status.
- Ensures that the resident, and the resident’s substitute decision-maker, if any, are consulted in the development, implementation, and progress of the restorative plan of care.
- Assesses new admitted residents for current physical and functional capacity to determine eligibility for restorative care program.
- Reassesses the restorative plan of care at weekly reviews that includes documentation.
- Completes monthly evaluation and as needed to ensure the resident’s restorative care needs and progress are documented and implemented in a timely manner.
- Tracks and documents residents’ attendance and participation in the Restorative Care Program.
- Develops and offer a variety of stretching, strengthening, and mobility exercises to residents at and structured times throughout the week to collaborate the Physiotherapist and Occupational Therapist (OT) as per resident needs and plan of care.
- Collaborates with therapists to ensure residents are properly using their assistive devices and sitting posture.
