**Position Summary**

Reporting to the Director of Care, the Professional Practice Clinician is responsible for the supervision and operation of quality nursing services in keeping with the standards of the Ontario College of Nurses and Long-Term Care standards. The Professional Practice Clinician promotes excellence in clinical care through leadership, evidence-based practice, and education.

**Qualifications and Skills Required**

Education
- Current certification and registration with the CNO, and who is a member in good standing
- Registered Nurse, Bachelor of Science in Nursing required and Masters Preferred.

Experience
- 3-5 years of experience working as a registered nurse in the long-term care sector

Interpersonal Skills/Contacts
- Ability to identify and respond to basic needs or requests. Tact and diplomacy are required to deal with minor conflicts or complaints. Requiring expert advice when resolving issues.

**RESPONSIBILITY**
- Decision making would have a moderate impact on the organization
- Modifies data, ensures accuracy, and prepares reports.

- Provides instruction to coworkers and coordinates some work of coworkers
- Knowledge of applicable LTC legislation, experience in working in a unionized setting and knowledge of over-seeing nursing staff scheduling
- Current First Aid/CPR
- Demonstrates Leadership Skills and Abilities
- Adult education and program management experience
- Knowledge of RAI/MDS
- Ability to work as a member of a team
- Human relations skills required to handle intercessions with residents/tenants, families, staff, and professionals
- Cultural sensitivity
- Proficiency in Microsoft Office (Word, Excel, PowerPoint, and Outlook)
- Current (within 6 months) Vulnerable Position/ Sector Screening (VPS)
- Legally entitled to work in Canada.

**Key Responsibilities**
- Provides nursing care in accordance with administrative policies, physician’s orders, established standards and nursing principles
- Develops and maintains current individual resident care plans through interdisciplinary care conferences and care planning
- Maintains accurate and complete clinical records on all residents according to established documentation policies and procedures
- Ensures proper recording and safekeeping of all drugs including controlled drugs and narcotics
- Ensures the proper maintenance of accurate confidential records and reports as required by the Ministry guidelines and by CNO documentation standards
- Monitors and controls, requisitioning of drugs and narcotics
- Participate in care conferences and assist with the admission of new residents and their families to aid in their orientation into the facility
- Maintains contact with residents to ensure adequate care and is available to families for consultation as needed
- Responsible for the preparation of daily duty assignments and reallocation of staff to meet resident care plans
- Liaise with Food Services/Dietary, Therapeutic Recreation, Supporting Housing, as required
- Organize and assist as needed with Mask Fit, Eye Clinic, Foot Care Clinic, and Hearing Aid Clinics
- Responsible for all notifications, documentation and medical records completed in the event of a resident death
- Attends Annual / Admission Review Conferences
- Assist and support frontline staff with professional development, when required, to ensure a higher standard of care
- Identify improvement strategies for staff compliance with documentation standards
