Caregivers Alberta is a registered charitable organization providing resources, support and education to help people caregiving for family members or friends. Our services are wide-ranging, as are the needs of caregivers: from our caregiver support line and one-on-one coaching to peer support groups and our award-winning COMPASS for the Caregivers education program. We serve caregivers and healthcare professionals across Alberta. We are continually expanding our programs and supports, ensuring our services properly respond to caregivers’ evolving needs. We also give caregivers the tools and resources they need to be more confident and resilient, and to ultimately enjoy the caregiving journey.

**Position Scope/Summary**:
Caregivers Alberta is seeking an engaging, compassionate, energetic, detail oriented, highly organized individual to join our team as the **Systems Navigator**. Reporting to the Manager, Support Services, you will connect caregivers with local community and government resources, activities and services to support the needs of their care recipient and improve the health and wellbeing of both caregiver and care recipient. Your responsibilities include system navigation, general peer support and validation, and discussing strategies around accessing services. There may also be the opportunity for family and public education as well as group facilitation. This role guides individuals to appropriate resources, programs, and services and provides critical front-line support to caregivers.

The Systems Navigator works collaboratively across the health and social support systems to connect individuals to services and community-based support that meet practical, social, and emotional needs of the caregiver and their care recipient. Every day you’ll identify what is affecting someone’s health and wellbeing; often discovering that it’s related to a gap in service around the needs of their care recipient. Your role will then be to connect them with local community and voluntary groups offering a range of activities and services. This could be services with healthcare such as day programs, home care, or respite services. Or social based activities such as art classes, gardening clubs or exercise groups. You may help them seek more practical advice, for example on debt or housing issues. You will advocate for the best interests and rights of clients throughout the entire working relationship to promote access to adequate resources and supports. You will work in a safe, efficient, and effective manner, to provide client-centered, strength-based and equitable services that empower caregivers.

**Support Navigation**
- Education around common caregiver challenges and the supports available to address those challenges
- Care and communication strategies that embrace and maintain quality of life for the caregiver and care recipient
- Navigation of community resources; and
- Support clients to access and engage with supports in the community.
- Make appropriate referrals and linkages to ensure clients have needs met; making warm hand offs to referral agencies.
- Educate and support client in completing required forms in accessing services
- Actively participate in sharing of digital referral link and promoting its use with other agencies.
- Co-facilitate caregiver support groups as required
- Facilitate education sessions like Lunch N Learns or program specific workshops as required
- Provide administrative support for caregiver support groups as required
- Proactively gather information of the latest resources across Alberta, applicable to caregiver needs

**Technical Responsibilities**
- Complete documentation of client services and notes in a timely manner utilizing Salesforce
- Maintain client confidentiality and adhere to all safety policies
- Proficient use of remote work platforms such as MS Teams, Zoom, and other virtual IT tools
