**Job Summary**:
Reporting to the Director, Care Design & Change Management, the Project Facilitator will work closely with the newly established Transformation Consultant team to organize, document and facilitate project activities, including supporting project delivery outcomes and project management to support transformation initiatives. This position will facilitate project and portfolio management reporting and will collaborate with multiple stakeholders, internal and external, to ensure outcomes are achieved. The Office of Health System Transformation is central to the successful implementation of LHSC’s new Strategic Plan, and leads a portfolio of projects across clinical operations that fulfill our vision to transform health, together. The Project Facilitator will support the Transformation Consultant team in designing, testing and evaluating transformation initiatives in collaboration with stakeholders across the organization and our health system.

The Project Facilitator will coordinate meetings for project work, capture and organize action items, and will engage in active follow-up to ensure project deadlines and deliverables are met. This position will also maintain project logbooks, schedules, stakeholder tracking, task assignments, and will create context-specific documents such as presentations and briefing notes as required. The Project Facilitator will play a key role in maintaining awareness of operational transformational plans, to ensure LHSC leadership, front-line staff, external stakeholders (including community organizations, partner hospital staff, and/or patients and caregivers) are collaborators in the design, implementation, and evaluation of transformation initiatives that align to LHSC’s strategic priorities.

**Regular Full-Time**

**Rate of Pay**: $33.64 - $42.05

**Hours of Work**: 37.5 hours per week

**Qualifications**:

- College Diploma in Health, Business, or Education, or acceptable by the hospital
- Certification: Project Management - PREFERRED****:

- Minimum of 2 years’ experience
- Demonstrated planning and evaluation skills
- Excellent facilitation skills e.g. consensus building and problem solving
- Excellent project and change management skills
- Proficient in financial management
- Excellent interpersonal and relationship management skills
- Demonstrated resource planning and performance measurement skills
- Advanced computer literacy including Microsoft Office Suite and corporate productivity tools
- Proficient knowledge of local and regional health care systems
