**About the Opportunity**:
Reporting to the Provincial Placement Manager, Continuing Care, the Placement Coordinator is responsible for managing the Long-Term Care (LTC) wait list as set out in policy by DSLTC. They monitor and maintain the LTC wait list in Seascape and manage facility vacancies using Pathways. Placement Coordinators match clients to appropriate vacancies for permanent admission. They inform and educate clients, families, service providers, acute care, continuing care staff and other partners on process and policy. They communicate with clients, families, and stakeholders (partners) during the placement process including but not limited to admission, transfer, respite, cost of care, etc. They also oversee temporary short stay respite admissions. They help negotiate the admission of complex clients and assist in problem solving with providers to attain a successful admission. They are the first line of communication in the refusal process from service providers, often negotiating to mitigate that refusal. They prepare requests for exception to the LTC policy and submit them to the Provincial Placement Manager. They orientate new Placement staff and educate new Care Coordinators to the placement process.
**About You**:
We would love to hear from you if you have the following:
- Bachelor's Degree in a Health related discipline requiring licensure
- Currently licensed with the applicable provincial licensing body
- 3-5 years recent related experience
- Experience in long term care assessment/placement an asset
- Demonstrates excellent written and oral communication skills.
- Good presentation skills.
- Knowledgeable about the principles of adult education.
- Demonstrates the ability to train others on policy and procedures.
- Demonstrates competence in the use of the windows operating system
- Demonstrates conflict resolution skills, strong facilitation skills and excellent negotiation skills
- Demonstrates collaboration and teamwork skills and leadership abilities
- Ability to use Microsoft Word, Outlook, Microsoft Teams, Excel.
- Excellent organizational skills to manage workload demands.
- Competencies in other languages an asset, French preferred

**Hours**:
- Permanent Full-time Position; 70 hours bi-weekly
- Shifts may include days, evenings, weekends and holidays
**Compensation and Incentives**:
$33.80 - $42.68 Hourly
