The Geriatric Outreach Team provides outpatient services to geriatric clients requiring tertiary mental health care. Working in team environment the Social Worker will provide high quality, safe, effective evidence-based care in partnership with patients, families and community partners.

The Social Worker on the Geriatric Outreach Team operates within an outpatient environment supporting both clinic-based appointments as well as community-based appointments with patients and their caregivers providing support in individual and group settings.

**Essential Qualifications**
- Knowledge of a Safety Culture in a Health Care Setting in compliance with the OHSA
- Registered with the Ontario College of Social Workers and Social Service Workers as a Registered Social Worker
- Masters of Social Work
- Strong clinical knowledge /skills in comprehensive geriatric assessment, individual and family counselling, conflict resolution and advocacy to individuals with serious and persistent mental illness and their families
- Knowledge of psychiatric diagnosis and treatments, psychotropic medications and symptom management
- Strong crisis intervention, advocacy, conflict management and facilitation skills
- In-depth knowledge of major mental illness from bio-psychosocial perspective
- Knowledge of Mental Health Act, Health Care Consent Act, PHIPA and other relevant legislation
- Comprehensive knowledge of and ability to mobilize community, health/mental health and social services
- Highly developed interpersonal and problem -solving skills as well as the ability to function effectively independently and with an interdisciplinary team
- Excellent organizational, time management and communication skills
- Demonstrated skills in providing education to patients, families and health care teams
- Familiarity with outcome measures related to clinical practice, ability to measure and evaluate practice change outcomes.
- Travel in catchment area required
- Supports research and innovation in practice, previous experience in research an asset
- Proficient computer skills with a variety of programs.

**Preferred Qualifications**
- Proficiency in French would be an asset
- Experience working with older adult clients and/or in mental health and addictions

**Teaching and Research**
- St Joseph's Health Care London through its affiliation with Western University and Fanshawe College is a leading research and teaching hospital. As an employee of St. Joseph's, you will be expected to engage in role related teaching and research activities in addition to any of your clinical duties.

**Immunization Requirements**
- Provide documentation you have received two doses of Covid 19 vaccine and a written commitment to follow any future Public Health recommendations and St. Joseph's Health Care London policies and processes related to Covid management in the workplace.
- Provide vaccination records of proof of immunity against measles, mumps, rubella and varicella (chicken pox)
- Provide documentation of the Tuberculosis skin testing.
