We are seeking **Home Support Workers** (HSW) for our newly established MGH2Home Program in **Toronto**. This is a 12 month contract position with guaranteed full-time hours. As an **HSW** you will assist patients with their basic personal care and activities of daily living as part of a multi disciplinary team supporting patient's ongoing independence.

**Job Description**:

- Assists with activities of daily living such as bathing, dressing, toileting, skin care, socialization and companionship
- Assists with the positioning of patients in bed or chair
- May assist and or supervise patient specific delegated exercise/activities to maintain mobility and independence
- Performing basic meal preparation and light housekeeping duties such as washing dishes, taking out the garbage and laundry for the patient. Sweeping and vacuuming within the patient’s immediate living area may be required.
- Provides short-term respite for medically stable patients while the primary caregiver is out of the home
- Maintains awareness of patient’s abilities, current diagnosis made by medical professionals, diet, personal interests and special needs

**Qualifications**:

- Supportive Care Worker Certificate, Health Care Aide Certificate or completion of Term 1 of a PSW program
- Current Level C CPR and Standard First Aid Certificate
- Current Vulnerable Sector Police check
- Must possess the physical ability to perform the day-to-day duties described in the duties and responsibilities
- Effective English communication skills, written and verbal
- Ability to interact with others in a caring, tactful and courteous manner
