**About YWHO Port Hope/Northumberland**
Rebound proudly leads the YWHO Port Hope/Northumberland Hub in partnership with NHH Community Mental Health, Community Living and Respite Northumberland, Community Health Centres of Northumberland, YMCA Northumberland, and the Municipality of Port Hope. The Hub offers an inclusive, welcoming space for youth to socialize and access a wide range of wraparound supports.
The Hub is part of the Youth Wellness Hubs Ontario (YWHO) provincial initiative. YWHO is an Integrated Youth Services (IYS) initiative designed to address the service gaps in the youth mental health and substance use sectors in Ontario. YWHO supports local service providers to work together to provide young people in Ontario aged 12 to 25 with low barrier access to a full range of services including mental health and substance use supports, primary care, education, employment, housing and other social services all through youth-friendly access points. YWHO services are designed to improve experiences and outcomes for youth by:
- Increasing access to rapid, low barrier services;
- Reducing transitions by providing care in one location; and,
- Providing tailored, high-quality programs co-developed with youth to meet their needs.
**Position Summary**
The Care Coordinator Manager will facilitate and support youth-led communications and marketing activities, represent and promote the Hub Network organization in the broader community, and be responsible for ongoing and specific projects as assigned.
**Roles and Responsibilities**
- Plan, develop, and implement the YWHO model and Core Components to promote barrier-free access for diverse youth.
- Establish and maintain strong working relationships with advisory committees.
- Lead service integration using protocols and Memorandums of Understanding.
- Oversee transitional case management for youth under 16 and rural youth seeking services.
- Monitor program quality assurance and lead continuous quality improvement projects.
- Manage daily operations, budget, staff, and community relations.
- Ensure safe operations in a busy drop-in environment and promote best practices in mental health, substance use care, and social services.
- Recruit, supervise, and manage a multi-disciplinary team.
- Orient new staff and partners to the YWHO model and its policies.
- Develop partnerships for coordinated care, ensuring seamless service pathways.
- Engage youth in developing service pathways and programs.
- Maintain a diverse schedule of support for youth wellness.
- Ensure youth, family, and community members’ voices inform Hub development and governance.
- Represent the Hub in the provincial YWHO Network, including attending YWHO Network Operations meetings and collaborating with the YWHO Provincial Office and the provincial Implementation and Evaluation team.
- Enable staff and partners to access YWHO training and capacity-building activities.
- Ensure compliance and privacy in data collection and input.
- Facilitate local evaluation data collection and measure selection.
- Support the collaborative development of the YWHO’s strategic goals and ensure the implementation of annual business plans and special projects.
- Oversee the adoption of the YWHO data collection platform, My Wellness Passport, the use of Measurement Based Care by employees and partner agency staff.
- Oversee the adoption of the YWHO Virtual Platform as the virtual services option within the Hub, work with YWHO Provincial Office to train staff on use of the platform, and support for implementation of the platform with youth outreach and service pathways.
- Ensure consistency among staff and service delivery partners in assigning client identifiers and overseeing data collection/data input processes, ensuring compliance and privacy within a Personal Health Information Protection Act (PHIPA) framework.
- Work with the YWHO Provincial Office to facilitate local evaluation data collection and selection of measures/indicators, as required.
- Providing reports to the Collaborative Partnership Team (CPT) regarding Hub operations and highlighting successes and potential risks.
