**Position:**:Care Manager (Full-time, Contract ending November 2025)

**Position Summary**:
Responsible for coordinating and conducting client assessments, problem identification, service co-ordination, education, advocacy, discharge planning, and organize and coordinate support groups for clients and families.

**Responsibilities**:
**Provide Care Management services**:

- Conduct assessments, prepare care and/or service plans, and provide care management for clients
- Provide crisis management, counseling and follow-up plan, as required.
- Identify and document occurrences and take corrective actions when required
- Maintain accurate records in the care plan, progress notes and statistical reports.

**Organize and coordinate support groups and related services**:

- Organize educational workshops through partnership with other agencies and/or health care professionals
- Supervise volunteers assisting in groups or individual support

**Education**:

- Post-secondary degree in a health-related field or equivalent education and work experience
- Health professional with membership in good standing with a regulatory body in Ontario preferred
- Valid First Aid and CPR certificate an asset

**Experience & Skills**:

- Two (2) years’ experience working with senior population
- Ability to communicate in Tamil, an asset
- Experience working with older adult population with mental health and/or addictions an asset
- Knowledge of assessment, counseling, care management and group work
- Excellent interpersonal, communication, decision-making and assessment skills

**Other**:

- Vulnerable Sector screening required
- A valid driver’s license and a car is required for home visits
- Occasional weekend and evening work may be required
