**Organization Overview**

The Peterborough 360 Degree NPLC is a dynamic community-based interdisciplinary primary care clinic providing comprehensive health services to residents of Peterborough City and County with particular expertise in providing care to people whose health is impacted by poverty, homelessness and poor access to the social determinants of health. The 360 Degree NPLC operates from a philosophy of harm reduction and is committed to working from within an anti-oppressive and anti-racist framework that is trauma-informed and which values cultural humility and inclusiveness.

The Safer Supply Program (SSP) provides community-led, flexible, low-barrier access to pharmaceutical medications through a health equity and harm reduction lens. This program offers primary care, alongside an interdisciplinary team that includes nursing, case management, health system navigation, and peer support to attend to the social and structural determinants of health impacting the lives of our clients. The goal of the program is to reduce the unnecessary deaths due to the unregulated and toxic street supply of drugs, and to provide low-barrier access to a safe alternative for people though relationship-based healthcare.

**We are looking for a Systems Navigator who**:

- Is passionate about working with people who use drugs and people who are unhoused and/or living in poverty
- Is a flexible and adaptive person who seeks leadership opportunities
- Is a critical thinker and creative problem solver
- Is committed to working from a harm reduction philosophy of care
- Believes in the value of client-centred, team-based care and enjoys working in an environment that empowers all members of the team, including the client, to contribute in meaningful ways
- Has energy and motivation to contribute to the planning, implementation, and evaluation of a new safer supply program in a medium-sized urban centre
- Enjoys being part of building something new, and has energy to provide leadership in program planning and development

**Role and responsibilities to include**:

- Develop positive, trusting, therapeutic relationships with clients through the sharing of lived/living experience
- Provide ongoing education and support on harm reduction and related services
- Provide clients with information about community services to ensure basic needs are being met, including emergency housing, food, and financial support
- Provide outreach services and accompaniment to appointments and community activities as needed
- Collaborate with lived/living experience workers from partner agencies, including PARN, CTS, and EFRY
- Inspire hope by sharing your own personal experience when appropriate
- Work as part of an interdisciplinary team, providing consultation to the SSP team
- Maintain client records and correspondence exercising strict confidentiality.
- Advocate for clients with colleagues, agencies, and institutions in the community as needed, including at the 360NPLC, hospitals, pharmacies, and client workplaces
- Support clients in being at the centre of their care, empowering them to make decisions on their health and goals
- Participate in huddles and SSP team meetings
- Help to create local programming to support people with lived/living experience
- Participate in quality improvement, research, and program planning, implementation and evaluation activities

**QUALIFICATIONS**
- **Lived/living experience of substance use is essential**. Additional lived/living experience of mental health and/or homelessness is an asset
- Experience working in a peer role (lived/living experience role) for 3+ years preferred
- Knowledge and understanding of harm reduction philosophies and practices required
- Strong ability to build relationships with individuals impacted by substance use, mental health and/or homelessness.
- Knowledge and understanding of community resources, preferably those available to people living in Peterborough City and County
- Emergency First Aid (or equivalent) and CPR level C (Health Care Provider level) preferred, but this can be done after obtaining employment
- Ability to set goals and get results within a set timeline
- Ability to work as a team and work independently as needed
- Effective written and oral communication skills
- Effective time management, decision making, and organizational skills.
- A good team member and supportive of interdisciplinary practice
- High level of confidentiality
- Flexible towards change
