**Roles and responsibilities**

We are currently looking to fill one **Permanent Full-time position** in **Norfolk County** for a blended position that is Part-time **Tiered Exercise Program (TEP) Facilitator** and Part-time **Personal Support Worker (PSW)**. Totaling **35 hours per week**.

**You may be eligible for the Ontario PSW Return of Service Incentive program. Recent PSW graduates could receive $10,000 for 12 months of service. For more information please visit : PSW Return of Service (healthforceontario.ca)**

**Please note: Access to a reliable vehicle is needed to drive between client sites.**

Personal Support Worker (PSW) Duties include the following:

- Assists consumers in their own homes with non-medical activities of daily living in accordance with individual service plans and with March of Dimes Canada policies and procedures:

- Personal care -Lifts and Transfers -Rising/retiring-Bowel and bladder procedures-Meal preparation and feeding-Range of motion exercises-Other light housekeeping duties.
- Performs duties in a professional manner, which respects the dignity and right to privacy and maintains consumer and staff health and safety.

Tiered Exercise Program Facilitator Duties include:

- Provide support to seniors in their own home with a 12-week program
- Facilitate, when needed, group exercise programs to seniors
- Assist with documenting their outcomes of the exercise program
- Responsible for tracking and reporting statistical data about the program and the client
- Work independently in the community and or with community partners

**Qualifications and experience**

Background / Qualifications:

- High School graduate or equivalent
- Current Standard First Aid and CPR certification
- Experience working with people with physical disabilities, experience/knowledge/understanding of Independent Living or Consumer based care.
- Knowledge of and abilities in transferring, lifting, bowel and bladder routines and other attendant care duties
- Physical Demands Self Analysis Required upon hire
- Good interpersonal and communication skills
- Ability to work flexible and regularly scheduled hours
