**Role Overview**
We are seeking an individual who can prioritize client needs, ensure their safety and promote a recovery-oriented and client-centered approach. By working within a quality improvement framework, you can continuously enhance the quality and outcomes of the services provided. Embedding the guiding principle of client and family-centered care into the work is crucial for improving the decision-making processes, enhancing health outcomes, and ensuring positive client experiences.
**About the Team**
AHFT utilizes an interdisciplinary and Housing First approach to support individuals who are homeless, living with mental health challenges, addictions and are involved with the mental health and justice system (referred through court support programs and/or local correctional facilities).
**Role Description**
The Housing Coordinator is responsible for providing housing coordination support to the Assertive Housing First Team, forging positive relations with landlords and overseeing the management and administration of the team’s rent supplements.
**Summary of Duties**:
- Provide Housing leadership and extensive support to service teams at Cota, and Loft At Home Program
- Oversee and manage a rent subsidy portfolio of 220+ rent subsidies. This will include preparing and updating monthly occupancy reports before submitting the reports to accounts payable
- Ensuring that notice of rent increases are applied correctly, deleting or adding units to the occupancy report, adjusting rent payments to ensure the correct monthly subsidy allowance is paid to each landlord
- Review and investigate any unit arrears and/or outstanding payments that are identified by the landlord
- Working collaboratively with the AHFT Team and client to secure housing.
- Ensuring client qualifies for our rent subsidy program. Confirm finance source. (ODSP/OW, CPP, OAS)
- Support the team to coordinate move-ins, ordering of basic start up packages (ordering of furniture, kitchen wares, bedding etc.)
- Attend weekly team meeting and other team meetings upon request
- Review Cota's service agreements between Cota, the tenant and Landlord and/or the lease between the Landlord and tenant (as applicable)
- Follow up with maintenance issue in units on behalf of tenants
- Establish and maintain partnerships with new and existing landlords, health care providers and community partners
- Provide education on the RTA and landlord/tenant issues to the service teams
- Support eviction prevention by collaborating with the service teams to resolve landlord/tenant issues and disputes
- Work with the service teams and other external committees to develop knowledge transfer and/or education on housing first model
**Summary of Qualifications**:
- A minimum of a Community College Diploma or post-secondary education related to property management, logistics, and/or administration
- A minimum of 2 years of direct or transferable experience in project coordination and/or administration
- Knowledge and appreciation of housing issues as applicable to the target population
- Knowledge of legislation and regulations including Residential Tenancies Act
- Able to build and maintain positive working relationships with new and existing landlords
- Able to work collaboratively with landlords to address any tenant related issues or disputes
- Good facilitation, crisis management and dispute resolution skills
- Good interpersonal and communication skills (verbal and written)
- Proficient with computer software programs including Word, Excel, Outlook, Internet
- Good judgment and ability to work independently as well as cooperatively with others
- Non-judgmental attitude
- Consistent/reliable and flexible
- Ability to recognize safety issues and adhere to safe work practices
- Demonstrated client-centred and customer focused approach
- Good problem solving skills
