**Key Responsibilities**:

- To work as a member of the inter-professional team.
- To provide direct client care and medication / treatment administration and document in Yardi Voyageur the care in compliance with regulatory standards and policies and procedures of Elmsmere Villa Retirement Residence and the (CNO) College of Nurses of Ontario.
- To process doctor’s orders and ensure proper administration of drugs and narcotics in compliance with all regulatory requirements, scope of practice, policies and procedure of Elmsmere Villa Retirement Residence and the CNO.
- To complete and ensure client’s clinical care assessments, care/ services plans and documentation are accurate, up to date and changes are communicated as required.
- Facilitate and provide client and family education. To plan, coordinate, manage & evaluate the resident’s care plan by working with the resident and his family (POA / SDM).
- To participate in conferences where applicable.
- To respond and manage significant changes in client’s condition and or medical emergencies; provide emergency first aid and CPR (cardio pulmonary resuscitation) and coordinate the emergency response alongside RPN or DOC
- To respond and inform resident and his family of any concerns. Ensure appropriate action taken and inform team, family physician and Director of Care in a timely manner. Documentation of the event is at the upmost importance in Yardi.
- To promote and uphold the organization’s values and philosophy relating particularly to ethics, morality and integrity towards residents and their families.
- To process strong written and oral communication skills, critical thinking, care coordination, time management. Must demonstrate sound clinical knowledge.

**Qualification**:

- **Current Basic Cardiac Life Support and First Aid certification required.**
- **Current vulnerable position sector screening (VPS) Criminal Record Check.**
- **Current TB test and redone every year.**
- **Fully vaccinated for Covid -19 as advised by Public Health**

**Education**:

- Accredited UCP course with a passing mark of 90% or higher, pass task orientated administration
- Understand the scope of UCP
- or Registered Practical Nursing Diploma

**Background**:

- Excellent communication skills; both in French and English
- Supervisory skills, emergency management and added nursing skills.

**Personal Requirements**:

- Ability to work as part of a dynamic team;
- Capacity to adapt to different situations and change;
- Keen interest in working with seniors

**Functions**:
Assists the Director of Care/RPN in the planning, development, organization, implementation and evaluation of the overall quality of resident care by the nursing department;
- Delivers and receives report at the beginning and end of each shift;
- Participates in the development of care plans for each resident and ensures that it is carried out, reviewed and revised as necessary;
- Performs initial and ongoing nursing assessments of residents’ needs along with the RPN;
- Carries out constant monitoring of residents for physical and emotional well-being and accounts for each resident on each shift;
- Administers or supervises medication as ordered by physician;
- Assists residents with personal care as required;
- Calls residents physician when necessary; and call family when necessary.
- Orders and administers medications and delivers treatment in compliance with pharmacy policies as well as the standards of the College of Nurses of Ontario, for current and new resident
- Liaises with other health care personnel, agencies and families as required;
- Assist in orientation of the residents, family members and nursing staff;
- Completes audits and reports as required;

**Function summary**

As a member of the interdisciplinary team, The Unregulated care provider (UCP) works alongside the Registered Practical Nurse (RPN) to deliver safe, effective, patient/resident-centered, ethical and evidence-based nursing care to patients/residents according to scope of practice and in accordance with Elmsmere Villa Retirement Residence Care policies, procedures and standards and with the College of Nurses of Ontario standards. The UCP communicates effectively, both verbally and in writing with patients/residents, families, colleagues and other health discipline members in the Yardi voyageur electronic documentation system.

**Preferred qualifications**
- Fluent in English (French as an asset)
