The RAH Social Work department is a fast paced exciting facility that provides opportunities to work, learn and grow within an interdisciplinary team. The work may include psychosocial assessment, crisis intervention, complex discharge planning, financial assessment, capacity assessment processes, education, treatment, advocacy, resource finding, consultation, making appropriate referrals both internal and external to the hospital and liaising with community agencies/programs. As a Social Worker II, you will be a member of an inter-disciplinary team and provides a range of social work services to patients and their families. Your services may include: psychosocial assessment, resource-based interventions, individual, group and family treatment, risk screening, counselling, case management and coordination, transition or discharge planning, and prevention, promotion and public awareness activities.

**Required Qualifications**:
