**Position Summary**

The LEAD Community Health Worker will work at UHN and is an integral member of the Gattuso Centre for Social Medicine’s clinical team. The LEAD Community Health Worker responsibilities include: Supervising, organizing, assigning and distributing workload among assigned Community Health Workers to ensure the department operates in a manner that is efficient and effective, coaching and mentoring assigned staff to foster a positive atmosphere that supports team building, collaboration and shared learning, performing administrative responsibilities, to facilitate the improvement of processes,/procedures; participating in professional development, teaching/educational activities, safety and quality improvement initiatives to ensure excellence in the provision of services and ensure Community Health Workers have the necessary equipment and resources, communicating and disseminating relevant information to team members and management personnel in a timely and consistent manner across departments. Understanding and departmental policies and procedures, adhering to Hospital and departmental policies and procedures while maintaining patient confidentiality, performing cross functional responsibilities as assigned or requested, performing other duties related to the job classification as required.

**Qualifications**
- At minimum, completion of a 2 year community college program in Social Service Work or recognized equivalent required. Masters degree in Social Work preferred.
- Membership with the Ontario College of Social Workers and Social Service Workers (OCSWSSW) required
- Minimum six (6) years practical and related experience
- Strong knowledge of, and familiarity with the principals of population health and health equity
- Leadership or project management experience preferred
- Experience in maintaining high quality practice in the clinical and community environment
- Ability to provide duties in a professional manner and produce high quality work
- Exercise and initiative and good judgement with the ability to multi-task
- Compliance with confidentiality requirements
- Effective organizational, communication, and interpersonal skills
- Knowledge of UHN and/or departmental policies
- Client service oriented with the ability with the ability
