Activation Coordinator

**Position Overview**
The Activation Coordinator is responsible for developing, coordinating, implementing and evaluating meaningful activation programs that meet Seniors and Aging patients’ intellectual, physical, mental and spiritual needs. The Activity Coordinator upholds the values of Compassion, Accountability, Integrity and Courage demonstrated in effective collegial relationships and inter-departmental partnerships; by ensuring a safe and comfortable environment; and by demonstrating courtesy in every interaction.
**Primary Responsibilities**
- Develop, coordinate, implement and evaluate leisure activity programs and services to meet the physical, mental and spiritual needs of all patients
- Ensure patients are given a wide array of activity choices including: arts & crafts, music events, avariety of exercises, cognitive exercises, library services, inter-generational activities, religious and spiritual activities and community events
- Coordinate the involvement of community groups, entertainers, school groups, community resources, etc.
- Complete an assessment for each new patients pertaining to social history, physical ability and behaviors, spiritual lifestyle, important family members and losses, family dynamics and contacts and language abilities as required
- Provide ongoing documentation of patients progress and changing needs by conducting one on one interview as required
- Coordinate and collaborate with the Volunteer program. Responsible for continual orientation of Volunteers and support with activation activities.
- Post and promote a monthly activity calendar
- Introduce new activities on a regular basis
- Coordinate the transport of patients to and from activities
- Regularly evaluate the success of the activities by seeking feedback using satisfaction surveys and other benchmarks
- Create an enjoyable, relaxed and entertaining environment for patients
- Attend daily huddles and minute rounds
- Conduct activation activities aligned with Hospital Elder Life Program objectives for Seniors in the
- Emergency, Medicine and Transitional Care Portfolios.
- Flexibility to accommodate role changes and additional duties
**Education**
- Certificate or diploma in recreation/activation required.
- Additional certificate in gerontology an asset.
- Current BLS required.
**Experience**
- 1 to 2 years of recent experience in recreation/activation coordination preferred.
- Previous experience working with seniors preferred.
- Familiarity with community resources an asset.
- Computer literacy required; demonstrated knowledge of Microsoft Office Suite.
- Familiarity with standards set out in Long-Term Care Home Act.
- Ability to articulate and demonstrate resident safety strategies.
- Demonstrated ability to be creative, pro-active, work independently as well as be an effective team member.
- Ability to empathize with patients and their families while demonstrating a high degree of patience, tolerance and confidentiality.
- Understanding of patients with cognitive impairment.
- Demonstrated excellent organizational skills, attention to detail and time management skills.
- Excellent interpersonal and communication skills both verbal and written.
- Good work and attendance record required.
