**JOB SUMMARY**:
We are currently seeking a RAI Coordinator to join our management team. The RAI Coordinator reports to the Director of Care (DOC) and is responsible for overseeing the RAI-MDS system and ensuring compliance with all applicable legislation and Ministry of Health requirements. Provides orientation and education to new hires with documentation software (e.g. PCC, Mede-Care) and RAI-MDS. Is a liaison with multidisciplinary team members for planning care.

**SKILLS AND KNOWLEDGE REQUIRED**:

- Registered Practical Nurse, in good standing with the College of Nurses of Ontario
- Minimum of three (3) years clinical, project leadership or management experience
- Proven ability to meet required deadlines
- Demonstrated success leading new initiatives
- Excellent oral and written communication skills
- Knowledge and experience with adult education strategies/facilitation
- Previous experience presenting to groups
- Well organized, with excellent time management skills
- Strong aptitude for detail-oriented work
- The ability to work with and relate to interdisciplinary team members and other internal stakeholders
- Strong problem solving abilities
- Proficient computer skills

**RESPONSIBILITIES**:

- Oversees the RAI process according to Home policies, MOHLTC regulations, College Standards of Practice and inter-RAI requirements.
- Ensures a schedule of assessments is developed, current and accessible to all RAI users.
- Monitors staff for accurate coding in the RAI-MDS and support validation of coding.
- Works with RAI users to develop processes that ensure data accuracy and quality.
- Promotes, supports and participates in quality improvement.
- Communicates with RAI users and management on a regular basis.
- Oversees the development and coordination of the data submission process.
- Audits assessments and care plans.
- Generates RAI-MDS reports and facilitates their use in clinical and operations decision making.
- Participates in appropriate scheduled meetings such as Professional Advisory Committee and provides input decisions that improve and refine resident care.
- Coordinates and attend weekly multidisciplinary care plan meetings.
- Assists in ensuring that resident care plans are initiated within 24 hours and that they are updated as required.
- All other duties as assigned by Director of Care or designate.
