**About the Role**:
As a **Youth Worker**, you will support Indigenous youth and families facing challenges such as mental health, addiction, and school or home difficulties. You’ll provide screening, brief assessments, and referrals to internal and external services, addressing social determinants of health to promote well-being and healthy living for Indigenous youth.

**Key Responsibilities**:

- Assist youth and families in navigating services and providing case management.
- Conduct screenings and brief assessments using established tools and protocols.
- Collaborate with youth and families to create navigation strategies, such as Individual Education Plans (IEP).
- Organize and lead case conferences to match client needs with appropriate services.
- Advocate for youth and families when they face barriers.
- Educate and raise awareness about mental health and addiction resources.
- Facilitate group activities that help connect and engage youth.
- Promote Indigenous values, beliefs, and traditions that enhance health and well-being.
- Maintain accurate client records and monitor referral outcomes.
- Evaluate program processes and outcomes.

**Qualifications**:

- Post-secondary education in Social Work, Youth Work, or a related field.
- Proficiency with Microsoft Office tools, especially Excel, Word, and PowerPoint.
- Ability to work flexible hours, including evenings and weekends.
- Clear Criminal Reference Check for working with vulnerable populations.

$50,000 with benefits
