**Brockville and District Association for Community Involvement -Resource Facilitator( Management)**

**Scope of the role**:
This position assists individuals living with an intellectual disability, and their families, in ways that will enhance the individual’s presence and participation in community life. Time is spent with the whole family and contact with them is maintained regularly to provide support in crafting a vision for the individual’s future, strengthening them on their journey and providing the necessary resources, tools, and guidance on a plan forward. The facilitator coordinates opportunities for families and individuals to learn from others and connect to and build larger family networks in their community.

**Responsibilities**:

- Assist families to recruit, screen and hire supporters for their family members and where appropriate supervise.
- Source and support opportunities for those supported to identify leisure interests, source places and spaces of those interests, participate in recreation, and identify volunteer opportunities and employment that are appropriate and valued.
- Support families to establish and nurture support networks, including next-generation and legacy supports.
- Assist the individual in developing and implementing strategies and supports that are reflective of person-centred planning, self-determination, choice, and citizenship.
- Source opportunities for learning in natural community settings.
- Foster and facilitate family groups to share information, discover new ideas and strengthen support among families for one another.
- Plan for and support the individual’s changing needs as a result of life transitions or needs related to aging, medical or psychosocial.
- Be actively engaged with families to assist in finding safe and suitable living arrangements
- Work collaboratively and serve in a leadership/mentorship role in an interdisciplinary team environment with other services and providers
- Facilitate and manage change, while being flexible, innovative, and creative
- Demonstrate excellent communications skills for interaction with staff, peers, board of directors, funders, persons supported, stakeholders and the public.
- Will work collaboratively with the family to motivate others, whether they are individual support staff, teams and/or volunteers to achieve desired goals
- Supervise and coach support staff and contracted support people in concert with the family.
- Participate in the “emergency after-hours on-call” rotation.
- Provide statistical and incident information for monthly and quarterly reports.
- Attend all required association training.
- Participation in projects and committees internal to BDACI and other community development efforts as required.
- Contribute content to BDACI’s website blogs and be active on BDACI’s social media platforms.
- Other duties as assigned.

**Efforts and Working Conditions**
- Work is performed both out of the office, in family homes, as well as the corporate office space
- Frequent travel within the Leeds-Grenville region with occasional travel out of the region for training, or other business matters
- Non-standard hours may be required (e.g., evenings and weekends); may be called into work for emergency purposes
- Occasional lifting of objects weighing approximately 50lbs
- Changes may be made to the list of families the employee works with or the tasks the person is asked to perform, and those decisions are based on the family’s and BDACI’s needs
