**Position Overview**
- Promote capacity building efforts and initiatives across areas of THP that care for unplanned orthopedic care patients with a focus on hip fracture patients
- Sustainably drive practice change, with a focus on improving access to and quality of clinical care, aligned to HQO Hip fracture Quality standards and other evidence-based innovative practices
- will be a part of delivering and help inform the care priorities at THP to support effective and timely patient transitions from ED to OR to acute care and then to post acute care
- Provide orthopedic care coaching and mentoring to staff and clinicians to build care competencies.
- Collaborate with colleagues across M-stie and C-site on service coordination/integration, and change management initiatives, using quality improvement methodologies to achieve results.
- Consult on clinical care when the complexity of the patients’ needs is beyond the existing competencies of providers.
- Provide support to Indigenous communities and Indigenous organizations for their implementation efforts, where Indigenous communities wish to be involved, in collaboration with regional partners.
- Achieving the highest quality of patient-centered care and exceptional experience that aligns with THP strategic goals of quality, access, and sustainability.
- Contribute to the formal evaluation of innovations and tests of change.

**Key Responsibilities**:

- Participate in rounds/huddles to ensure that there are conversations related to hip fracture patients currently in the ED, Surgery or on the unit to actively advocate for this patient population.
- Foster collaboration and contribute care integration.
- Build competency amongst front-line providers in the delivery of acute othopedic care.
- Create learning opportunities and use appropriate teaching/learning strategies, materials and technology to facilitate the learning of the inter-professional team at the bedside when it comes to unplanned orthopedic care with a focus on hip fracture patients
- Lead/co-lead the development of patient/care-partner facing materials using a co-design with patient advisors.
- Guide care innovations and practice change initiatives for the hip fracture patient population specifically across THP.
- In collaboration with CLs/CQCLs, consult/serve as a direct care provider for complex situations, as appropriate
- Lead/Co-Lead working groups that are working on leveraging technology to bring the care pathway to life in the electronic health record (in Epic)
- Monitor patient progress against care pathway milestones until discharge from the orthopedic/surgical care units at M-site and C-site.
- Liaise with physicians, interdisciplinary professional team and care partners to ensure that all care transitions are timely and effective.
- Monitor and addresses patient and family satisfaction with care, health and safety issues along with the physical environment for patient care to meet our standards for best practices

**Qualifications**
- Graduate of a recognized College or School of Nursing and preferably hold a Baccalaureate degree
- Registered in good standing with the College of Nurses of Ontario (CNO) or currently meet the eligibility criteria to register in good standing in the Temporary Class or General Class with the CNO
- Advanced clinical preparation with a minimum of 3 years of sound clinical experience in surgery/orthopedics.
- Recent related experience in ED, acute care, discharge planning is an asset
- Previous leadership experience preferred
- Previous experience leading or participating in clinical quality improvement initiatives
- Strong communication skills along with the ability to establish positive relationships/partnerships with all staff, patients, families, internal and external partners
- Demonstrates initiative, creativity and innovation in clinical practice
- Excellent interpersonal, communication, organizational and decision making skills
- A demonstrated commitment to patient centered care
- Ability to promote change and support and mentor others through the change process
- Demonstrates continuous learning and professional development
- Well-developed computer skills
