- Minimum two to four (2-4) years change management experience, or experience with coordinating new initiatives within the health system
- Change Management Professional designation is considered an asset
- Demonstrated knowledge and experience of the healthcare sector, in either a clinical role, or through an industry connected to the health system
- Previous working knowledge of the Home and Community Care sector and having awareness of how various clinical roles operate in home care and the greater health system is considered an asset
- Awareness of PHI and the importance of privacy and security in the health system
- Satisfactory attendance

- Excellent oral / written and interpersonal communication skills
- Strong interpersonal skills and the ability to handle difficult situations with tact and diplomacy
- Ability to work effectively within a team and independently
- Strong skills with computer systems including all Microsoft office tools

**ACCOUNTABILITIES (Major Responsibilities of the Position)**
- Ensure the alignment of change management activities with participating health service providers, to smooth the adoption process and contribute to problem solving resulting in successful implementation.
- Incorporate adoption and change management strategies based on situational awareness of the details of the change and the groups being impacted.
- Design, lead and implement a communication and change management strategy for the "people side of change", which includes education and training of health care professionals within the local health system.
- Conduct implementation activities, document lessons learned and present findings in a logical and easy-to-understand manner.
- Promote and deliver training sessions for health care services collaboratively with health care professionals working with palliative care patients and other Ontario Health Teams.
- Work with health care professionals from different organizations to gain buy-in for change.
- Conduct process mapping, lead meetings and presentations, support overall project management.
- Respond to partner inquiries and provide referrals or solutions.
- Meet targets and deadlines and adapt to project changes and needs.
- Have a positive and supportive service attitude and work well within a team.
- Obtain feedback and liaise between Ontario Health, other Ontario Health Teams, and local partners to meet project objectives
- De-escalating conflict with stakeholders when necessary
