**About the Opportunity**:
Reporting to the Provincial Placement Manager, Continuing Care, the Placement Coordinator is responsible for managing the Long-Term Care (LTC) wait list as set out in policy by DSLTC. They monitor and maintain the LTC wait list in Seascape and manage facility vacancies using Pathways. Placement Coordinators match clients to appropriate vacancies for permanent admission. They inform and educate clients, families, service providers, acute care, continuing care staff and other partners on process and policy. They communicate with clients, families, and stakeholders (partners) during the placement process including but not limited to admission, transfer, respite, cost of care, etc. They also oversee temporary short stay respite admissions. They help negotiate the admission of complex clients and assist in problem solving with providers to attain a successful admission.
