Position Overview Working as a collaborative member of the inter‑professional team, the Nurse Practitioner (NP) functions in an expanded role, demonstrating a high level of autonomy and expert skill to formulate clinical decisions and appropriately manage acute/chronic illness and wellness promotion for adults and seniors. Senior’s Health Clinics The NP leads the provision of comprehensive geriatric services in the GAIN (Geriatric Assessment and Intervention Network) Clinic. This model of geriatric outpatient care employs inter‑professional comprehensive geriatric assessment (CGA) for managing frail seniors and rapid assessments for post‑discharge patients from inpatient and ED settings. The NP provides direct patient care focused on health promotion, maximizing patient safety and function to support frail seniors living at home. Outreach With a growing seniors population in Scarborough, supporting seniors at home is a priority. As an expert geriatric NP, you will provide outreach virtual and/or onsite consultation support to frail senior patients in LTC homes, retirement homes and the community (including home visits as part of GAIN assessment). You will obtain critical information from care teams in LTC, retirement homes or community settings and provide specialized geriatric consultation to support care plans and interventions. Consultation Liaison Services The NP will provide consultation services for frail elderly patients with multiple comorbidities and complex medical regimes in hospital, working with inpatient and ED teams as a member of the consultation liaison service. You will provide expert knowledge and support to inpatient and ED teams and collaborate with BSO and psychiatry liaison (CL) teams as necessary. This position is ideal for a Nurse Practitioner with a passion for geriatric care. Hours will require flexibility and will include evening and weekend work, and may include home visits. The successful candidate will be required to provide proof of liability coverage. Requirements Masters of Nursing Current registration in the Extended Class with the College of Nurses of Ontario 3 years recent clinical work experience with frail older adults as a Nurse Practitioner Certification in Geriatric Nursing from Canadian Nursing Association preferred Demonstrated ability to exercise independent judgment and to assess situations and problems efficiently and effectively Effective verbal and written communication skills Strong interpersonal, organizational and problem‑solving skills Ability to work well within an inter‑professional team and to foster effective working relationships Ability to exercise initiative and observe strict confidentiality Proven good attendance record with the capability of maintaining this same standard Demonstrates values consistent with SHN’s Code of Conduct and is committed to fostering a healthy and positive work environment in accordance with Health & Safety policy and legislation Utilizes protective equipment provided and identifies and reports hazards Engages in ethical conduct and strives to understand and appreciate the diversity of our patient/staff population and community Excellent work performance and attendance record Asset Specialty certification an asset Membership in professional organizations preferred Ability to speak another language Accommodation And Diversity Statement Scarborough Health Network (SHN) embraces and celebrates our community’s unique multicultural heritage and diversity.
