Care & Transitions Facilitator – Hemodialysis Unit The Care & Transitions Facilitator (CTF) is a results-oriented practitioner within the In‑centre Hemodialysis, Kidney Care Centre (satellite unit) and Apheresis programs. The CTF role focuses on enhancing the quality of patient care in a complex health, social, and fiscal environment, advancing initiatives such as new patient education, supporting accreditation priorities, and engaging patients and families. The CTF also contributes to project-based work including the Nursing Model of Care Review, SLED/CRRT work plan, and unified strategic objectives with Anti‑Racism, Equity and Social Accountability (ARESA) and the Indigenous Wellness Team, driving BIPOC and equity‑focused improvements for patients receiving hemodialysis care. Shift Schedule: 8‑hour rotating shifts on days and evenings. Key Responsibilities Lead safe and seamless transitions for patients in outpatient hemodialysis and apheresis settings, promoting person‑centered care and effective resource utilization. Coordinate care planning, transition, and evaluation with interdisciplinary teams, ensuring clear treatment goals and schedules. Manage utilization of clinical resources for exceptional and complex patients. Communicate effectively with medical, interdisciplinary, and unit teams and patients/families to enrich patient progress. Coordinate care delivery with internal and external stakeholders, liaising with outside agencies for timely medical, functional, and rehab services. Advocate for needed patient services to support safe and effective discharge/transition. Negotiate with patients for limited services or space when necessary. Identify and resolve patient care/safety issues, communicating concerns to the Clinical Manager. Collaborate with support service teams to optimize clinical processes and patient transitions. Leverage external resources to reduce system barriers and facilitate safe and timely discharges. Act as a resource for patient care issues, problem‑solving with cross‑program and community teams to ensure efficient access and flow. Other Duties Educate patients, families, students, residents, and new staff. Coordinate multiple referrals. Network and build internal and community partnerships. Identify and lead unit‑based quality initiatives. Qualifications Bachelor of Science in Nursing or related health discipline; registration with a regulatory college in good standing. Master’s degree considered an asset. Current and advanced clinical assessment skills in hemodialysis and apheresis. Demonstrated problem‑solving ability. Proven success in generating case‑management ideas. Excellent communication, facilitation, and negotiation skills. Ability to facilitate teamwork among multidisciplinary professionals. Strong planning, organization, and coordination skills. Proficiency with computers and experience interpreting data. NOTE: All unionized and non‑unionized candidates are welcome to apply.
