Clinical Pharmacist – Inpatient Unit or Clinic Unity Health Toronto, comprising Providence Healthcare, St. Joseph's Health Centre and St. Michael's Hospital, serves patients, residents and clients across primary and tertiary care. Duties & Responsibilities Provide direct patient care in the management of drug therapy to achieve positive patient health outcomes, primarily for patients of either a surgical or medical‑type inpatient unit or clinic, and coverage to other areas as needed. Review medication orders; assess patient pharmaceutical needs; identify, resolve and prevent drug‑related problems; educate patients; ensure continuity of care; and provide drug information. Promote accurate, safe, appropriate and cost‑effective use and distribution of drugs throughout the hospital, including weekend, evening and on‑call rotations. Support and participate in educational and research programs sponsored by the pharmacy discipline or hospital. Participate in the development, revision and implementation of standards, protocols, procedures, and guidelines pertaining to drug therapy and pharmacy practice. Qualifications Registration as a pharmacist with the Ontario College of Pharmacists. Hospital Pharmacy Practice Residency or equivalent experience. Minimum of 1‑year relevant clinical experience in providing pharmaceutical care to patients in a teaching hospital setting, with medical‑surgical experience preferred. Commitment to student and patient teaching; preferred experience. Excellent communication, interpersonal, organizational and problem‑solving skills and demonstrated ability to function in a team environment. Interest in research involvement and support.
