Job Title: Behavioral Health RN Care Manager Department: Quality Reports To: Clinical Quality Director SUMMARY: The Behavioral Health RN care manager provides direct and indirect patient care services in an integrated behavioral health setting. This role focuses on supporting individuals with mental health and/or substance use disorders through assessment, care coordination, patient education and collaboration with multidisciplinary teams to promote patient stabilization and recovery. ESSENTIAL DUTIES AND RESPONSIBILITIES: Conduct comprehensive nursing assessments of patients presenting with behavioral health needs, including mental health and substance use concerns. Formulate and implement a care plan that addresses the patient’s identified needs by assessing the patient/family needs, issues, resources and care goals; determining the choices available to individual patients; educating the patient/family on the choices available. Establish a care plan that is mutually agreed upon by the health care team and the patient/family. Plans will contain specific mutual self‑management goals, objectives, and interventions with the patients that are action oriented. Identify barriers and empower patients/families to achieve maximum levels of wellness and self‑management. Involve patient/family in the formation and ongoing evaluation of the plan of care. Monitor and evaluate patients’ physical and mental status; identify changes in condition and escalates as appropriate. Support medication management, including administration, monitoring side effects, and reinforcing adherence, completing medication reconciliation. Support crisis intervention, de‑escalation, and safety planning, including assisting in psychiatric emergencies. Collaborate with behavioral health providers, primary care teams, case managers, and external agencies to coordinate holistic patient care. Document all nursing assessments, interventions, patient responses, and communications in the electronic health record (EHR) in a timely and accurate manner. Serve as a patient advocate, supporting recovery‑oriented and trauma‑informed care practices. Identify social determinants of health needs and effectively utilize community resources to meet these needs. Facilitate referrals, follow‑up appointments, and transitions of care including ED and inpatient stays. Collaborate with providers, other healthcare team members (including inpatient facilities, the patient’s payer and health system administrators) to facilitate transitions of care across the healthcare continuum and optimize clinical and financial outcomes. Perform all duties and responsibilities in accordance with basic principles and guidelines of professional nursing. Participate in regular team meetings and appropriate quality and organizational committees. Participate in the of new personnel. Abide by the organization’s compliance program and requirements. Work collaboratively with leadership team to improve and enhance care delivery through the evaluation, development and enhancement of policy and procedures. Drive improvement of clinical quality measures—in partnership with the rest of the quality department. Perform and/or participate in other appropriate projects or duties as assigned; this may include vaccine clinics, home visits or other off‑site nursing needs. Requirements: Graduate of an accredited School of Nursing. Current, unrestricted Registered Nurse license in the state of Ohio. A minimum of 2 years’ experience in behavioral health, psychiatric nursing or substance use treatment required with a focus on youth and adults. PMH‑BC certification.
