This position supports the delivery of accessible, high-quality, and culturally grounded mobile mental health services to individuals living with serious mental illness across Manitoulin Island.Due to the specialized nature of this position, the Mobile Wellness Team Lead must have an in-depth understanding of treatment pathways for serious mental illness. The incumbent ensures holistic, person-centred care that integrates both traditional and western healing approaches, promotes collaboration among interdisciplinary team members, and fosters partnerships with community and health-care organizations to enhance service access and continuity of care.ResponsibilitiesProvide day-to-day leadership and operational oversight for the Mobile Wellness Team, including intake, care coordination, and clinical service deliveryParticipate in policy development, program planning, and continuous quality improvement initiativesSupport team members (e.g., navigator, social workers, nurses) through case consultation, mentorship, and clinical guidanceCoordinate service delivery schedules, ensuring efficient use of resources and timely responses to client needsEnsure new referrals are addressed promptly and appropriatelyMonitor and evaluate program outcomes and quality indicators to ensure alignment with organizational and funder expectationsProvide consultation and support for complex cases, including crisis situations and mental health or addictions concernsSupport the integration of traditional and cultural healing pathways in collaboration with Elders, Knowledge Keepers, and Traditional HealersUphold principles of trauma-informed, holistic, and culturally safe careEnsure services are delivered in accordance with ethical, legal, and professional standards of practiceStrengthen partnerships with local health and social service agencies, hospitals, and First Nation communitiesParticipate in community outreach, education, and promotion of the Mobile Wellness ProgramRepresent Noojmowin Teg Health Centre at interagency meetings, community committees, and collaborative initiativesSupport coordination between Mobile Wellness services and other internal programs to ensure seamless care pathwaysMaintain accurate documentation and data entry within the Electronic Medical Record (EMR)Compile reports, statistics, and outcome data as required by funders and the organizationEnsure adherence to organizational policies, procedures, and professional standardsSupport onboarding, supervision, and professional development for new and existing team membersPerform other duties as assignedQualificationsMandatoryCurrent registration and good standing with a recognized regulatory college (e.g., Ontario College of Social Workers and Social Service Workers, College of Nurses of Ontario, College of Registered Psychotherapists of Ontario, or other relevant body)Bachelor of Social Work degree, or another health-related degree with relevant experienceMinimum of 3 years of clinical experience in mental health and/or addictions servicesDemonstrated leadership, supervisory, and coordination experienceValid Class G driver’s licence, access to a reliable vehicle, and clear driver’s abstractClear Vulnerable Sector Criminal Record CheckPreferredExperience working with First Nation communitiesFamiliarity with electronic medical record systemsKnowledge of community resources and service providers within the Manitoulin Island regionAbility to speak Anishinaabemowin considered an assetKnowledge, Skills, And AbilitiesStrong leadership and team facilitation skillsKnowledge of serious mental illness, crisis intervention, and trauma-informed careUnderstanding of Anishinaabe culture, traditional wellness, and Indigenous models of healthExcellent interpersonal, organizational, and communication skillsAbility to work collaboratively across disciplines and with minimal supervisionWork ConditionsServices delivered across seven First Nation communities and regional sitesFrequent travel and occasional evening or weekend work may be requiredCombination of office-based, community-based, and mobile service delivery environmentsSeniority levelMid-Senior levelEmployment typeFull-timeJob functionHealth Care ProviderIndustriesHospitals and Health CareReferrals increase your chances of interviewing at Noojmowin Teg Health Centre - a place of healing by 2xGet notified about new Wellness Manager jobs in Little Current, Ontario, Canada.Little Current, Ontario, Canada 3 days agoWe’re unlocking community knowledge in a new way.
