Together, they create personalized plans that improve outcomes and ease the transition to home.Job SummaryWe are hiring a Speech Language Pathologist (SLP) who wants to be part of transformational change in home and community care by providing client-centered services. You will be part of a self-directed, collaborative team that conducts joint care planning focused on client-centered goals across physical, cognitive, social, emotional, and spiritual domains, creating holistic care plans. Each client is assigned a primary care clinician who leads care planning and facilitates the interdisciplinary team. As part of the SE Transitions team, you will be accountable for the care and outcomes you provide to your clients.Independent Contract or Employment options are available.Why join our team?Competitive compensation.
