Job Overview The Care Manager/“Designated Care Manager” is responsible for providing the highest degree of quality care and services to a consistent group of residents and their families in our assisted living/long‑term care and reminiscence neighborhoods. The manager delivers hands‑on care, physical and emotional support as outlined in each resident’s Individualized Service Plan (ISP), and maintains a safe, comfortable home‑like environment. Responsibilities & Qualifications Resident Care Review, read, note, and initial Daily Log to document pertinent resident information. Participate in development and monthly updates of ISPs. Manage designated group of residents; physically check on them throughout the shift. Observe, report, and document changes in condition such as skin, behavior, alertness, weight, diet, and activity participation. Attend daily Cross‑Over meetings led by the Lead Care Manager. Notify supervisor and Resident Care Director of increased care needs. Respond promptly to call bells and reset them as needed. Greet guests, family members, residents, and teammates and answer phone calls courteously. Assist with continence management and dispose of all products properly to maintain sanitation. Communicate with families and serve as a resource when needed. Help residents maintain independence, physical safety, and dignity in line with Resident Rights and Sunrise Principles of Service. Lead and assist residents with activities of daily living (ADLs) and encourage participation in activities indicated on ISP. Engage residents in life‑skill and reminiscence activities throughout the day. Maintain a warm, calm environment and ensure safety regulations are followed. Practice standard care precautions for cleanliness, hygiene, and health. Host and engage in daily resident activities. Risk Management & General Safety Ensure compliance with national and provincial occupational health and safety regulations. Perform safety procedures always, including PPE, fire extinguishers, Safety Data Sheets, and Lockout‑Tagout. Report all accidents, incidents, and unsafe conditions immediately. Maintain equipment and environmental hazards out of‑way and by fire code. Safely handle potential exposure to bloodborne pathogens and hazardous materials. Ensure oxygen tanks are stored safely and monitored for prescribed levels. Housekeeping & Laundry Services Clean and maintain resident rooms: bed making, trash removal, laundering, and floor care. Wash resident laundry as noted in ISP and general housekeeping tasks. Wash and fold dining room linens and napkins. Maintain common areas in a clean, tidy manner. Dining Service Serve meals in the dining room and support setup and cleanup. Promote a pleasant dining experience for all meals. Participate in pre‑meal meetings. Follow resident diet plans and confidentiality diet board. Document dining habit changes in the daily log. Provide room service delivery when requested. Practice safe food handling, adhering to universal care precautions. Reminiscence Neighborhood Care Manager Responsibilities Encourage residents in life‑skill and enrichment activities per ISP. Facilitate afternoon social gatherings. Integrate “Creating Pleasant Days” into daily routines. Blend multi‑sensory experiences into resident days. Participate in monthly letters home and letter writing with residents and families. Training & Team Success Commit to team goals and demonstrate the Team Member Credo. Contribute to engagement programs and participation in team surveys. Attend regular meetings: Town Hall, Department, Cross‑Over, and others as directed. Complete required training modules by Assisted Living Coordinator, Resident Care Coordinator, or Reminiscence Coordinator. Maintain compliance with infection control, bio‑hazard procedures, and all regulatory training. Perform other duties as assigned. Core Competencies Calm in difficult situations. Manage multiple priorities. Effective written and verbal communication. Organizational and time‑management skills. Good judgment, problem‑solving, and decision‑making. Experience & Qualifications High School diploma or GED (required per state/regulation). CPR and First Aid certification (state‑required). Minimum age 18. Preferred: prior experience working with seniors. Desire to serve and care for seniors. Ability to act in resident’s best interest. When driving a Sunrise vehicle, sign the driver job description and complete safety compliance.
