CBI Home Health serves a variety of clients and populations (i.e. adults & children), offering comprehensive triage services, holistic speech, language, swallowing assessments, and personalized interventions. Responsibilities (In‑Person or Virtual Delivery) Assessment and implementation of SLP interventions (i.e. swallowing assessments, dysphagia management, communication, etc.) in a home‑based setting Use of a collaborative and interdisciplinary team‑based approach to care and services Support a holistic and restorative approach to care (i.e. enhanced stroke pathway; cognitive communication supports, etc.) Leverage our digital health platforms to support care delivery, collaboration, documentation, and intervention Qualifications Registered as a member in good standing with the College of Audiologists and Speech Language Pathologists of Ontario Valid driver’s license and vehicle insurance Focus on person and family‑centered care Comfortable delivering care both in person and using virtual methods/technology Passionate, empathetic, use good judgement and apply critical thinking when making client decisions Excellent communication skills and ability to work collaboratively within an interdisciplinary team and independently Flexible and demonstrated critical thinking skills applicable to a variety of situations Valued role in keeping clients at home where they want to be About CBI Home Health CBI Home Health is committed to providing a diverse, equitable, and inclusive workplace where everyone feels safe and accepted as their authentic selves.
