Clinical Pharmacy Manager – Montefiore Health System Location: Yonkers, New York Hours: 8:30 AM – 5:00 PM (Day Shift); Pay Range: $152,000 – $190,000 Position Summary The Ambulatory Clinical Pharmacy Manager in Cardiology will lead clinical pharmacy services across outpatient cardiology programs. This role will oversee the design, implementation, and maintenance of Collaborative Drug Therapy Management (CDTM) protocols and provide direct patient care in cardiology clinics, including heart failure, hypertension, lipid management, post‑MI, atrial fibrillation, and anticoagulation clinics. The pharmacist will collaborate closely with cardiologists and other providers to optimize therapy, reduce cardiovascular events, and improve population health outcomes across the health system. Key Responsibilities Program Leadership – Develop, implement, and maintain ambulatory CDTM protocols for cardiology disease states in alignment with institutional policy and state regulatory requirements; serve as the primary CDTM pharmacist for disease state management; expand CDTM services across additional ambulatory sites and support credentialing of other pharmacists under the protocols; monitor outcomes and develop key performance indicators (KPIs) to assess impact on quality measures. Direct Patient Care Responsibilities – Initiate, titrate, and discontinue medications under CDTM protocols (e.g., beta‑blockers, ACE inhibitors, SGLT2 inhibitors, statins, DOACs); order and interpret labs (e.g., lipid panels, renal function, INR) to guide pharmacotherapy decisions; provide longitudinal medication management and close follow‑up to ensure patients reach therapeutic goals; educate patients on chronic disease management, adherence strategies, and medication safety; coordinate care transitions such as post‑hospital discharge optimization and follow‑up. Collaboration and Education – Oversee and mentor clinical pharmacists providing ambulatory cardiology services across affiliated clinics; collaborate with physicians and care coordinators to integrate pharmacy services into care pathways; serve as a preceptor for pharmacy residents and APPE students, providing clinical instruction, mentorship, and evaluation during the cardiology ambulatory rotation. Regulatory Compliance and Reporting – Participate in cardiology ambulatory quality initiatives focused on cardiovascular health; ensure adherence to CDTM regulatory requirements, proper documentation, and risk mitigation strategies. Research & Performance Improvement – Lead and support research initiatives related to cardiovascular pharmacotherapy in ambulatory settings; design and conduct performance improvement (PI) projects aligned with hospital goals, regulatory requirements, and national standards; support pharmacy residents and students in research projects, publications, and presentations related to cardiovascular pharmacotherapy; maintain awareness of emerging literature, guidelines, and best practices to inform program development and evidence‑based interventions. Eligibility Requirements Education & Licensure Doctor of Pharmacy from an ACPE‑accredited college of pharmacy. Valid NYS Pharmacist license and registration. Completion of PGY2 Residency in Ambulatory Care or Cardiology highly preferred. Board certification in Ambulatory Care (BCACP) or Cardiology (BCCP) preferred. Experience Prior leadership or supervisory experience preferred. Experience in academic or teaching hospitals is desirable. Experience in research/performance improvement is desirable. Knowledge & Skills In‑depth knowledge of cardiovascular pharmacotherapy. Excellent analytical, organizational, communication, and teaching skills.
