Collaborating closely with an interdisciplinary team, you will develop tailored care plans and interventions that empower patients to reclaim their independence and thrive in their daily lives. CBI Home Health serves a variety of clients and populations, providing comprehensive triage, holistic assessment and personalized interventions. With our fully integrated and secure digital health platform, you will have access to innovative tools, technology and interdisciplinary teams to support you in delivering our hybrid care models. You will work with clinical leaders, supervisors, therapist assistants, best practice specialists and students to support the safe and joyful delivery of high-quality, personalized care. Responsibilities Assessment and implementation of Occupational Therapy interventions (e.g., ADL/IADL strategies, equipment recommendations, home modifications) in a home-based setting Collaborate with an interdisciplinary team to deliver care and services Support a holistic and restorative approach to care (health restoration, coaching and self-management support) Leverage digital health platforms to support care delivery, collaboration, documentation and intervention What You Need To Be Successful Registered in good standing with the College of Occupational Therapists of Ontario Valid driver’s license and vehicle insurance Proof of Professional Liability Insurance Commitment to person- and family-centered care Comfort delivering care in person and via virtual methods Strong communication skills, teamwork and independent work ability Flexible with critical thinking applicable to various situations Value keeping clients at home where they want to be About CBI Home Health If you are passionate about person-centered care, this is your opportunity to contribute.
