Requisition Details:Employment Status: Permanent, Part-Time (0.6 FTE)Program Name: South West Ontario Therapy Program - Sarnia/LambtonWork Schedule: Days, Evenings, WeekendsOn Call: YesJob Summary:The Occupational Therapist (OT) is responsible for providing therapy services to individuals in the community within the framework of VON policies, procedures, standards, and quality and risk management. The OT collaborates with families, caregivers, healthcare professionals, and community services to deliver client-centered, evidence-based therapy services.New Graduates Welcome to ApplyKey Responsibilities:Assess, plan, evaluate progress, and document patient care plans based on occupational therapy standards and evidence-based practices.Evaluate patients' ability to function safely in their home and community environments, including ADLs and equipment needs.Assess cognitive, mental health, social, spiritual, and physical factors impacting independence.Develop client-centered care plans collaboratively with patients, families, and healthcare teams.Provide education and recommendations to patients and families regarding therapy goals.Assist with funding applications for equipment and home modifications.Monitor patient outcomes and adjust care plans accordingly.Document patient progress and care plans in electronic health records as per guidelines.Communicate effectively with funding agencies and team members regarding patient status.Manage caseloads and respond to changes in patient needs.External and Internal Relationships:Collaborate with interdisciplinary teams to meet patient care needs and therapy goals.Work with community services to ensure safe transitions and goal attainment.Education, Designations, and Experience:Master's in Occupational Therapy or equivalent prior to 2008.Valid registration with COTO and liability insurance.Membership with Ontario Society of Occupational Therapists is preferred.Experience in healthcare settings and with diverse patient diagnoses.Knowledge of community resources and healthcare system.Skill Requirements:Proficiency with technology and documentation tools.Strong communication skills (written and verbal).Ability to work independently and as part of a team.Adaptability to various social and economic environments.Effective time management skills.Commitment to professional development and community service.Other Requirements:Personal vehicle, valid driver's license, and insurance.Willingness to travel and support various sites.Criminal Records Check and other background checks as required.French language skills are an asset.Ability to wear PPE.Work conditions include attention to detail, physical activity, and good body mechanics.VON Canada is committed to accessibility and accommodations for candidates with disabilities.
