Position Summary Provides home support services to clients, including assistance with activities of daily living, delegated tasks as trained, meal planning and preparation, light housekeeping. Position Responsibilities Follow care plans and maintain accurate client records using appropriate documentation. Maintain effective relationships with clients to promote physical and emotional independence. Understand and perform delegated tasks and responsibilities. Communicate changes in client condition to supervisor and team members. Proactively seek and support client safety; consult Nurse Supervisor for medical and ethical dilemmas. Qualifications Required Comfortable traveling to Port Moody. Community Health Worker or Health Care Aide certified with BC Care Aide Registry. Availability for 2‑hour shifts daily. Experience in healthcare and Current First Aid (Standard) / CPR Certificate (Level C). Valid driver’s license and vehicle. Current immunization record including TB test completed within past 12 months required upon hire. Current vulnerable sector check screening required upon hire. Availability to work days or evenings, including weekends. Able to work independently and as part of a team with strong organizational skills. Effective interpersonal, communication, problem‑solving, and conflict‑management skills. Basic computer/smartphone skills an asset. Physically capable of lifting up to 55 pounds and performing required movements.
