Sunrise of Killarney Job Overview The Wellness Nurse is responsible for monitoring the health and well-being of residents and provides leadership support and assistance with resident care and services. Responsibilities include the overall well-being of residents, assisting in training and monitoring medication care administration, communicating with resident support services and families, and maintaining clinical quality assurance while leading and demonstrating the Sunrise Mission: to champion the quality of life for all seniors. This role aligns with federal, state/provincial, and local standards and Sunrise policies to promote high-quality care and services. Responsibilities & Qualifications Essential Duties As part of the Sunrise team, supporting our Mission, Principles of Service and Core Values is fundamental. The unique responsibilities for this role include the essential functions listed below: Coordination of Health Needs Identify ongoing needs and services of residents to promote high-quality resident services. Ensure proper follow-through and participate with evaluation and documentation for residents with changes in health care services and status. Assist Resident Care Director (RCD) in conducting resident evaluations and data gathering at move-in, 30 days post-move-in, semi-annually, and with status changes. Conduct monthly wellness visits for all residents. Monitor and follow up with residents who have experienced any fall, accident, or incident; document and report as required. Document pertinent information in the resident wellness file. Maintain communications with residents’ families or responsible parties to ensure medical needs are met. Contact residents’ attending physicians when necessary or upon family request. Ensure weights and vital signs are obtained monthly for each resident. Maintain wellness files according to Sunrise policies and federal/state regulations. Maintain medical supplies and emergency kits for the community. Provide clinical support and assistance to community team members as needed. Understand and follow infection control practices and promote infection control standards with the RCD and community team, including resident testing, training, and skills checks. Practice proper handling, disposal, and exposure control for bloodborne pathogens and infectious materials in compliance with universal precautions. Medication Programs Monitor each resident’s medication and treatment profile to ensure administration per orders and accurate documentation. Transcribe physician orders and MAR/TAR to include initials, date/time noted, and date faxed to pharmacy. Complete MAR/TAR audit each month. Demonstrate knowledge of the Six Rights of Medication Pass (Right resident, medication, dosage, time, route, right to refuse). Act as liaison with pharmacy services to ensure effective resident services. Monitor Medication Room, carts, and treatment carts for neatness, availability, and expired medications. Complete Medication Observations and skills checks for medication care managers as directed by the RCD. Quality Assurance and Regulatory Compliance Demonstrate knowledge in key quality improvement areas: Resident Centered Care Model, APIE, Quality Care Indicators and Outcomes, Sunrise Quality Assurance and Clinical Meetings. Know and apply all Federal, State/Provincial, and Local regulations related to resident care and services, Resident Rights, and the Resident Assessment/Evaluation process per regulatory requirements. Review Physician Reports for all new residents as directed by the RCD (note: ensure no duplicate entries). Order medications and equipment as directed by the RCD. Demonstrate knowledge of Sunrise practices and guidelines in resident care programs (skin, nutrition, behavior, falls, bowel/bladder). Assist in implementing, training, and monitoring documentation related to residents’ clinical health and wellness. Report resident changes, concerns, and solutions to the RCD as appropriate. Contribute to engagement programs and processes, including team member engagement surveys and improvement planning. Attend regular meetings (Town Hall, Department Team, Cross Over, and others as directed). Maintain compliance with required training and regulations applicable to the role. Perform other duties as assigned. Core Competencies Ability to handle multiple priorities. Strong written and verbal communication skills; ability to teach and guide team members to provide quality clinical care. Proficient in organizational and time management skills. Good judgment, problem solving, and decision-making skills. Experience and Qualifications Graduate of an approved college/school of nursing. Current state/provincial license as LPN/LVN. Minimum of one year of experience in home health, assisted living, hospital, or long-term care. Knowledge of nursing practices, techniques, and health regulations at federal/state/local levels. Strong assessment skills. Proficiency in computer skills, Microsoft Office, and Sunrise applications with ability to learn new applications.
