LVN Charge Nurse at Hamilton Focused Post Acute Care Partners FPACP is a dynamic company dedicated to delivering excellent long‑term care. We are looking for a Charge Nurse to join our team. Position Summary To provide nursing care to residents as prescribed by the physician and in accordance with standards of nursing practice, regulations, directed by the Director of Nursing. To lead or direct licensed and non‑professional staff in the delivery of direct resident care and support functions. Responsibilities/Summary of Essential Functions Lead and direct nursing staff and non‑professional personnel. Ensure resident safety and deliver best possible nursing care. Plan, organize and direct nursing functions of patients in the unit. Make nursing assignments appropriate to skill level. Maintain acceptable standards of patient care, document nursing history, perform assessment. Use assessment information to develop care plans and communicate to incoming personnel. Follow policies, procedures, infection‑control, and maintain confidentiality. Support and enforce infection‑control policies and procedures. Monitor vital signs, assess changes, notify physician. Respond appropriately in urgent and emergency situations. Assist in education, coaching, and positive work climate. Qualifications Current LVN/RN license. CPR certification required. Strong verbal and written communication in English. Ability to complete assignments in a timely manner. Flexibility and positive attitude with intermittent interruptions. Physical Requirements Ability to safely perform movements such as pushing, pulling, lifting, bending, kneeling, reaching, and lifting up to 50 pounds with or without reasonable accommodations.
